Antecedentes: En México son escasos los estudios encaminados a establecer la prevalencia de la rinitis alérgica y la dermatitis atópica en adolescentes tardíos.Objetivo: Determinar la prevalencia de rinitis alérgica y dermatitis atópica en adolescentes tardíos y compararla conforme el sexo.Métodos: Estudio transversal de 1992 adolescentes de 15 a 18 años, reclutados mediante muestreo probabilístico por conglomerados, estratificado y aleatorizado. Las prevalencias de rinitis alérgica y dermatitis atópica se identificaron con el cuestionario propuesto en The Internacional Study of Asthma and Allergies in Childhood.Resultados: Se incluyeron 1056 mujeres (53 %) y 936 hombres (47 %). La prevalencia de rinitis alérgica fue de 9.0 % (IC 95 % = 7.8-10.4); la frecuencia fue mayor en las mujeres (6.7 % versus 11.1 %, p = 0.001). Los síntomas de rinitis más conjuntivitis predominaron en las mujeres (16.0 % versus 23.1 %, p < 0.0001). La prevalencia de dermatitis atópica fue de 5.2 % (IC 95 % = 4.3-6.2); fue más frecuente en las mujeres (7.7 % versus 2.4 %, p < 0.0001). Los principales factores asociados con rinitis alérgica y dermatitis atópica fueron sexo femenino y atopia familiar (p < 0.001).Conclusiones: Se registró diferencia significativa en la prevalencia de rinitis alérgica y dermatitis atópica conforme el sexo en los adolescentes tardíos; las mujeres fueron las más afectadas. La atopia familiar fue otro factor asociado.

Background: The Notch signaling pathway plays an important role in regulating human immune function, but the relationship between allergic rhinitis (AR) and Notch signaling remains unclear. Objective: To investigate the role of Notch signaling in the pathogenesis of AR and its regulation on Foxp3-Treg cells. Method: The sera of 100 patients with AR and 50 controls were collected to assess the differences in Notch1, Jagged1, and DLL1 (Delta-like 1) expression. Experimental mice were divided into normal control, AR, Notch inhibitor, and dexamethasone groups. Allergic symptoms, total IgE levels, and the proportion of Treg cells in the peripheral blood were detected. Notch1, Jagged1, NICD (Notch intracellular domain, also known as ICN), and Foxp3 expression and Th1/Th2/Th17-related cytokines in the spleen were detected and compared between each group of mice. Results: Compared with the control group, the expression of Notch1 and Jagged1 in patients with AR was significantly elevated (p < 0.05). The expression of Notch1 and Jagged1 in patients with severe AR was higher than that observed in the mild to moderate AR patients and positively correlated with the levels of allergen sIgE (p < 0.05). The animal experiments revealed that compared with the normal control group, the expression of Notch1, Jagged1, and NICD in the AR group was increased, Foxp3 expression was decreased, and the proportion of Treg cells was decreased (p < 0.05). Compared with the AR group, allergic symptoms and total serum IgE levels and the expression of Notch1, Jagged1, and NICD were significantly decreased in the Notch inhibited group, whereas the expression of Foxp3 and the proportion of Treg cells were increased significantly (p < 0.05). The Th2-type immune responses were also enhanced and Th1-type immune responses decreased in the AR group, but the Th1/Th2 imbalance was reversed in the Notch inhibited group. Conclusion: Notch signaling downregulates Foxp3 expression and inhibits the differentiation of Treg cells to promote the development of AR. Blocking Notch signaling may be a potential treatment for AR.

Allergic Rhinitis in Pre-School Children Living in Urban Settings of the Altai Region: A Population-Based Cross-Sectional Study

Background.It is necessary to study the prevalence and risk factors of allergic rhinitis (AR) among pre-school children in order to develop a disease prevention strategy.Objective.Our aim was to study the prevalence, clinical and allergological features, and risk factors for AR in pre-school children living in urban settings of the Altai Region.Methods.At the screening stage, the study enrolled children aged 3–6 years attending pre-school educational institutions in 5 cities of the Altai Region. AR symptoms were determined using the ISAAC questionnaire. The AR was diagnosed if ≥ 2 symptoms (rhinorrhea, nasal breathing difficulty, itching in the nasal cavity, repetitive sneezing) lasted ≥ 1 h with a positive prick test and/or a blood level of specific IgE > 0.35 kU/L to at least one allergen (total 11).Results.The prevalence of AR in urban children aged 3–6 years (n = 3,205) was 10.6%; 48% of them were previously diagnosed with AR. 85% of children had a persistent course of the disease; 69% had mild AR. Most often, there was established sensitization to house dust mites (61.6%), birch pollen (40.9%), and cat fur (19.4%). The risk factors for AR were family history of allergies [odds ratio (OR) 4.2; 95% confidence interval (CI) 3.5–5.9], masculine (OR 2.8, 95% CI 1.9–4.0), smoking parents (OR 1.8, 95% CI 1.2–2.9), nonadherence to a regimen/dosage of vitamin D3 intake in infancy (OR 1.8, 95% CI 1.2–2.8), presence of asthma-like symptoms (OR 10.2, 95% CI 7.2–14.5), and manifestations of atopic dermatitis (OR 6.0, 95% CI 4.2–8.5).Conclusion.AR occurs in every tenth pre-school child (mainly of mild severity and persistent course), every second disease among them was diagnosed for the first time. Sensitization occurs to typical for childhood allergens. The risk factors for AR are family history of allergies, masculine, passive smoking, ignoring the recommendations of taking vitamin D3 in infancy, the presence of atopic dermatitis, and asthma-like symptoms.
