Prevalence and chemotherapy-induced reactivation of occult hepatitis B virus among hepatitis B surface antigen negative patients with diffuse large B-cell lymphoma: Significance of hepatitis B core antibodies screening

Abstract: The study concluded that anti-HBc screening is mandatory before chemotherapy. HBsAg-negative/anti-HBc-positive patients should be closely observed for signs of HBV reactivation through the regular monitoring of ALT. Prophylaxis lamivudine is recommended for anti-HBc positive patients before chemotherapy.

“…20 patients whose HBV DNA level and ALT were normal but HBV serological detection were abnormal (12 was HBsAg+HBcAb+HBeAb+, and 8 were HBsAg+HBcAb+HBeAg+), they also received preventive entecavir treatment. All the patients with HBV infection were analyzed for HBV serum test and the active infection patients were analyzed HBV DNA content before every cycle and all the patients needed to underwent regular analyses of blood and liver (especially ALT) and kidney function ( Table 2 ) [ 20 , 21 ].…”

“…The association between HBV and non-Hodgkin lymphoma has been well investigated[ 181 , 182 ]. In a study conducted by Elbedewy et al[ 182 ] in Egypt, HBV DNA was detected in 5/72 (6.94%) of patients with diffuse large B-cell lymphoma who were positive for anti HBc (191).…”

“…23,24 Individuals with negative HBsAg assay are not completely free of the risk of reactivation of hepatitis since viral DNA and HBcAb may persist as an occult hepatitis B infection (OBI). 25 That is to say, patients with HBsAg seronegativity who infected HBV previously indicates the clearance of natural HBV infection. But to a certain degree, virus replication may remain without HBsAg seropositivity.…”
