Spanish and immigrant children and adolescents vary widely in their frequency of dental visits and occurrence of dental problems. The aims of this study were to discover the prevalence of dental problems and utilization of dental services in the Spanish and immigrant child population, identify the type of treatment received, and analyze the socioeconomic and demographic variables which are associated with dental problems and non-regular utilization of dental services, based on data from the 2017 National Health Survey in Spain. The sample consisted of 4568 children aged between 3 and 14 years old. Utilization of dental services and dental problems were assessed against socioeconomic and demographic characteristics using logistic regression models. The prevalence of caries in Spanish children was 9.29% compared with 18.58% (p < 0.001) in their immigrant counterparts. The most common reason for dental visits was a check-up (Spanish: 65.05%; immigrants: 54.94%). In both groups, from the age of 7, there was a lower probability of non-regular utilization of dental services, although this increased when the social class was lower. The probability of presenting dental problems was lower in Spanish children living in towns with over 10,000 inhabitants and was higher, in both groups, over the age of 7 and in lower social classes.

Introduction. Dental caries is the most common oral health disease of school-aged children around the world. In this study, we aimed to assess the prevalence and associated factors of dental caries in primary schoolchildren in Yasuj township, Iran. Methods. In this cross-sectional study, a total of 460 children aged 7–12 years were investigated. Dental examination was performed at school according to the World Health Organization criteria. Sociodemographic data were collected using a structured questionnaire, and caries statuses of children’s teeth were recorded through a dental chart. Data were analyzed using summary statistics, chi-square test, and logistic regression model with odds ratio. Results. The prevalence of dental caries in primary, permanent, and whole dentition among children was 75.3%, 41.1%, and 89.8%, respectively. Among all considered factors, the caries presence in primary teeth was inversely (p<0.001) and in permanent teeth was positively (p<0.001) associated with the children’s age. Moreover, the odds of decaying permanent teeth were significantly higher in girls, in rural children, and in children whose fathers were not an employee compared to their counterparts (p=0.04, p<0.001, and p=0.02, respectively). Conclusions. The prevalence of dental caries among the studied primary schoolchildren in mixed dentition was high and associated with their sociodemographic factors. Providing and implementing preventive, therapeutic, and informative programs for controlling dental caries at individual, family, and school levels are necessary for local health policymakers.

Subject. This article analyzes existing theories about the prevalence and causes of dental caries, which is considered as a civilization disease. The crucial role of endogenous factors in the risk of caries, including a pulp and dental fluid, is noted. Data are given that acids do not actively participate in the process of demineralization, which is confirmed by numerous theoretical and hypothetical points. The role of some materials produced by dental industry based on this “pseudo-acidogenic” theory and used in the treatment of caries, as well as the false and negative consequences created by them in general, is analyzed, and adequate parallels are carried out. The purpose of the study was the theoretical justification of the key role of acetylcholine (the cholinomimetic mediator located on the Toms fibers) in the occurrence of dental caries. Methodology. Russian and foreign research works due to the etiology, prevalence and pathogenesis of dental caries were studied, and a comparative theoretical analysis of the available data on this issue was carried out. Results. Based on the results of numerous studies cited in the available literature and our studies, we performed an analysis of theoretical principles and substantiated the data that the process of demineralization occurring in dental caries cannot be caused by acids. Therefore, the only and decisive factor in the origin of the demineralization process is the occurrence of the process because of alkaline substances. Conclusions. Based on the studied references, we present the data that the most current theories of the etiopathogenesis of caries are imperfect in terms of solving the problem, which, in our opinion, requires further study of the process both of acetylcholine and the enzyme acetylcholinesterase inhibition in the local form and inside dental hard tissues.
