Tobacco, cannabis, and alcohol consumption, found to be most common among college students, is known to cause life-threatening diseases, and is correlated with social, financial and health problems. For the present study, we aimed to assess the sociodemographic factors affecting tobacco, alcohol, and cannabis consumption among university students. A cross-sectional study was conducted from January to March 2020, on a random sample of 507 undergraduates, between the ages of 18 and 24. The research instrument was a self- administered questionnaire with questions on the sociodemographic characteristics, and questions regarding the consumption of tobacco, alcohol, and cannabis.The mean ± standard deviation age of study sample was 21.56 ± 1.81 years, and 56.4% participants were females. The lifetime prevalence of tobacco usage among the study sample was 66.7%, alcohol 54.2%, and the cannabis had a prevalence of 13.8%. About 46.2% (n = 234) were co-users of tobacco smoking and alcohol, and 12.6% (n = 64) were co-user of tobacco smoking, alcohol consumption, and drug use. Analyzed with Pearson's chi-squared test, there was no statistically significant difference between students from private and public institutions on lifetime, in the past 1 year/1 month of tobacco smoking, alcohol consumption, and cannabis use (p > 0.05). The logistic regression model for dependent variable cannabis abuse in the past 1 year is associated with lower odds among female students (odds ratio: 0.337, 95% confidence interval: 0.167–0.682; p = 0.002). Smoking, drinking, and cannabis were found to be highly prevalent among university students. These findings can help program managers and policy makers devise effective and appropriate control programs and policies for substance-using university students.

Background Tobacco and areca-nut use among adolescents has been reported from different parts of India. Multiple factors influence initiation of tobacco use among adolescents. Initiation of one product gradually extends to multiple products. Studies on initiation lack documentation of the pathways and experiences post-initiation, which is required to holistically understand behavior patterns of adolescents for planning timely intervention. This study was conducted to trace pathways and identify factors influencing the initiation and continuation of tobacco and areca-nut among adolescents. Methods In this two-staged study, we conducted 14 focus group discussions (FGDs) with 166 adolescents studying in grades 7–9 (11–18 years) from six municipal schools in Mumbai, India. They had self-reported areca-nut or smoked or smokeless tobacco (SLT) use. Pathways of initiation were traced through in-depth interviews for 60 adolescents. Results Four multi-linear pathways of use were identified: (1) areca-nut only, (2) areca-nut to tobacco, (3) initiation with SLT, and (4) initiation with smoking. Raw or sweetened areca-nut, betel leaf, gutka, masheri, mawa, hookah, cigarette, bidi, and e-cigarettes were products reported to be used. Curiosity, easy access to tobacco products and normalization of tobacco use influenced initiation. Areca-nut acted as a precursor to tobacco use. Conclusion Tracing pathways in tobacco use helped to understand reasons for initiation, influences in continuation of use, and experiences of significance to the adolescents. Experiences of adolescents play a critical role in shaping the pathways of tobacco use. Understanding the pathways and influencers will further help to build effective health promotion communication, policies for sale to minors, and school-based cessation interventions. Implications Findings of the study provide an insight into unknown areas of information regarding products used by adolescents, their patterns of consumption, perceptions, and their pathways of initiation and continuation of primary and secondary products. This will help in developing specific public health awareness messages, policies regarding packaging and sale of areca-nut to children and interventions targeted for the adolescents and their specific products of use not just in India but for the South-East Asia region as areca-nut and tobacco use among adolescents is common in the region. The exercise of tracing the pathways provides basis for cessation counseling among adolescents.

Severity of age-related macular degeneration at first presentation in Bhutan: a 3-year national study

Background Medical services are still developing in Bhutan. There is no published national report on age-related macular degeneration (AMD). We therefore aim to determine the demographic characteristics and severity of AMD at first presentation among Bhutanese patients attending their recently inaugurated vitreoretinal (VR) clinics over a 3-year national survey, and to inform national health policy to develop suitable health program to prevent AMD-related blindness and visual impairment. Methods A retrospective cross-sectional consecutive case series study was conducted on all new AMD cases in Bhutan. If a patient presented with asymmetrical AMD, the eye with more severe AMD was considered. If both the eyes had the same severity one eye was chosen randomly. Collection of demographic data and clinical details including diagnostic testing (fundus photography, OCT and fluorescent angiography) and clinical staging were performed. Results Of 521 new AMD patients aged 71.9 ± 11.3 years, 306/521 (58.7%) were males (p = 0.005). At their first presentation, 234/521 patients (44.9%) already had late-stage AMD. Importantly, 69/234 patients (29.5%), that is half of total neovascular AMD (nAMD) patients, had disciform scars (DS) which were beyond treatment, and 7/234 patients (3.0%) had geographic atrophy (GA). Seven patients had retinal pigment epithelium tear at presentation. Fourteen of nineteen polypoidal choroidal vasculopathy (PCV) patients were younger than 50 years. Conclusions Half of nAMD cases presented as DS not amenable to the treatment. Many potentially treatable nAMD patients had already lost central vision and were legally blind. Young people with PCV losing vision early in life with longer morbidity-affected life and socio-economic burden was concerning. GA and DS cases need visual rehabilitation to improve their QoL. Incorporating a screening program for AMD with effective health education, and maintaining a national AMD Registry, would potentially lower AMD-related blindness and visual impairment.
