Prevalence and Prognostic Significance of T-Wave Inversions in Right Precordial Leads of a 12-Lead Electrocardiogram in the Middle-Aged Subjects

Abstract: Background-T-wave inversion in right precordial leads V 1 to V 3 is a relatively common finding in a 12-lead ECG of children and adolescents and is infrequently found also in healthy adults. However, this ECG pattern can also be the first presentation of arrhythmogenic right ventricular cardiomyopathy. The prevalence and prognostic significance of T-wave inversions in the middle-aged general population are not well known. Methods and Results-We evaluated 12-lead ECGs of 10 899 Finnish middle-aged subjects (52%… Show more

“…Moreover, they all correlate with the high prevalence of cardiac disease in the HRCE group. Compared to a healthy population, where only 1.2% had any type of Twave abnormality, our rate of 16% with T-wave inversions is high [24]. In our series, 8% had a widened QRS-T angle, a four-fold higher prevalence than in the general population (2.0%) [25].…”

“…This pattern of ''domed'' STE in the right precordial leads may be seen in adolescent and young adult males of Afro-Caribbean heritage, but is not common otherwise (8). PJTWP is more often found in women (4,9,10). Similar to other repolarization abnormalities, PJTWP is seen more often in patients of Afro-Caribbean heritage (6,(10)(11)(12).…”

“…5 The prognosis associated with inverted T waves in right precordial leads did not differ from the rest of the population, but inverted T waves in leads other than V 1 -V 3 and/or aV R were associated with a threefold risk for SCD. These changes were present in 0.5% and 0.7% of the population, respectively.…”
