The aim of the present study was to find the rate of caesarean deliveries and various indications of the procedure. Methods: The present study was conducted in the Department of Obstetrics and Gynecology, Mamata Academy of Medical Sciences, Hyderabad, on women who underwent LSCS over a period of 4 months. Out of the 445 deliveries, 200 were delivered by cesarean section. All the mothers who underwent LSCS in this hospital in the designated period were included in this study. Results: The LSCS prevalence was 44.94% in our institution. We have assessed the common indications of LSCS performed in this study population. In our study group, the range of age varies from 18 to 43 years with an average of 27.37±4.21 years, 30% of the women aged between 18-25 years, majority, i.e., 60% belongs to the age group of 26-35 years and only 10% observed in the higher age group. Out of the 200 cases, parity was distributed almost equally in this study group, i.e., 45% were primigravidae, and 55% were multigravida. 5% of the LSCS occurred at <32 weeks of gestation and 25% between 32-36 weeks. The majority of the LSCS were delivered between 37-40 weeks (70%) of gestation. Mode of conception was spontaneous for 97%, and 3% received infertility treatments. Fetal distress was an indication in 40 cases (8%), Breech presentation in 6%, PPROM in 5%, CPD in 4%. Other indications such as preeclampsia, placenta previa, unfavorable cervix, multiple gestation, abruptio placenta, LSCS on demand etc., was 1%. Conclusion:Although LSCS indications seen in our institute are the same in most institutions worldwide, efforts should be made to focus on reducing the primary LSCS rates thereby reducing the most common indication of previous LSCS in subsequent pregnancies.

A bstract Introduction Pandemic influenza H1N1/09 emerged for the first time in April 2009 and has spread widely across India since then. The number of cases have increased over time with the increasing need for respiratory support, causing significant morbidity and mortality. We evaluated the clinical course and outcomes of patients infected with Influenza A (H1N1) admitted to three multidisciplinary intensive care units (ICU) in Chennai. Materials and methods We performed a combined retrospective and prospective observational study of all patients admitted with H1N1 pneumonia at three multidisciplinary ICUs in Chennai from October 1, 2018, to January 31, 2019. Data including demographics, risk factors, and clinical courses were recorded. Outcome data including mortality was tracked up to 28 days. Results A total of 167 patients were admitted during the study period of which 154 were included in this analysis. The mean age of presentation was 58.2 ± 15.6 years and 59.1% of them were males. The mean acute physiology and chronic health evaluation (APACHE) IV and sequential organ failure assessment (SOFA) scores were 62.8 ± 23.2 and 5.8 ± 3.9 respectively. Oxygen delivery devices were required in 25.3% for a mean duration of 26.5 ± 5.7 hours. Non-invasive ventilation or high-flow nasal cannula (HFNC) was needed in 33.1% of patients for 59.9 ± 64.5 hours. The proportion of patients requiring mechanical ventilation was 41.6%. Rescue measures in the form of proning, use of inhaled nitric oxide (iNO), and extracorporeal membrane oxygenation (ECMO) were initiated for refractory hypoxemia in 26.6%, 14.1%, and 6.3% respectively. The mean duration of ventilator support was 8.5 ± 8 days. Tracheostomy was required in 20.3% of patients and 7.8% were ventilator dependent at 28 days. The mean ICU and Hospital length of stay were 8.3 ± 10.3 and 12.2 ± 14.1 days respectively and overall 28-day mortality was 20.1%. Conclusion A significant proportion of H1N1 patients admitted to the ICU required high-level respiratory support including non-invasive ventilation (NIV), HFNC, or invasive ventilation. Deployment of rescue therapies was common and the overall mortality rate was similar to those reported from Western countries. Background Cesarean section (CS) is one of the most common surgical procedures performed on women globally, and its prevalence has been rising significantly over the past few decades. CS rates have been increasing globally, raising public health concerns due to the associated financial burden and increased health risks compared to vaginal delivery. Methodology This study involves a retrospective analysis of delivery records from a tertiary care hospital in Uttar Pradesh, India, over 10 years, from January 2011 to December 2021. Results The data presented shows a significant shift in delivery methods over the past decade, particularly in the increasing rate of CS at our tertiary care center. From 2011 to 2021, the rate of CS increased from 39.6% to 52.4%. A detailed examination of the indications for primary elective CS reveals that oligohydramnios with intrauterine growth restriction (IUGR) remained the most common indication, increasing from 31% in 2011 to 34.3% in 2021. Other significant indications include malpresentation and cephalopelvic disproportion. Conclusion The study indicates a rising trend in CS rates at the tertiary care center, with various clinical and non-clinical factors influencing this increase. The increasing rates of maternal requests for CS, both primary and repeat, point towards a shift in patient preferences and expectations. Additionally, the changing trends in indications for both elective and emergency CS highlight evolving clinical practices and an increased emphasis on maternal and fetal safety. Continuous monitoring and analysis of these trends are essential to ensure that the indications for CS are appropriately managed, balancing the benefits and risks associated with surgical deliveries.
