Prevalence of colistin resistance of Klebsiella pneumoniae isolates in Iran: a systematic review and meta-analysis

Abstract: Objective Klebsiella pneumoniae is a gram-negative pathogen common cause of nosocomial infections. Colistin is a last resort antibiotic to treat infections caused by K. pneumoniae. In recent years, the resistance rate to colistin has increased in K. pneumoniae. This study evaluated the prevalence of colistin resistance of K. pneumoniae isolates in Iran using a systematic review and meta-analysis. Method A systematic search was performed for relevan… Show more

“…Although polymyxins were disfavored due to their potential nephrotoxicity, they are currently used in hospitals and are known as the last line of defense for the clinical treatment of multidrug-resistant Gram-negative bacterial infections. However, as treatment with polymyxins is becoming more common, reports of polymyxin resistance (including chromosomal and plasmid-mediated resistance) are increasing, especially in areas with high polymyxin use [ 6 , 7 ].…”

“…Resistance to TET was reported at 32% in China [ 34 ], 19% in Canada [ 32 ], and 41.5% in Brazil [ 6 ]. COL is a last resort antibiotic to treat infections caused by K. pneumoniae , and the resistance rate to this antibiotic has increased in recent years from 4.8% in 2013–2018 to 8.2% in 2019–2021 [ 35 ]; our resistance rate still higher, when compared to this finding and might come from the transmission between strains as this antimicrobial, was used for treatment calves with diarrhea. CIP, ENR, PIP, IMI, and STX were highly sensitive in our study, which could be a good choice for mastitis treatment, especially CIP, PIP, and IMI, as these drugs had never been used before at studied farms; this finding was supported by other studies in Brazil [ 6 ], in China [ 31 ].…”

“…Hence, studies of colistin-resistant Klebsiella isolates are increasing in number. According to a recent study by Narimisa et al [ 21 ], there was an increment in the level of colistin resistance by K. pneumoniae during the period of 2013 to 2021, which may be attributed to the increased utilization of this antibiotic in recent years. A strong relationship was observed between the increased resistance to colistin and carbapenem-producing K. pneumoniae [ 21 ].…”
