Prevalence of Human Immunodeficiency Virus and Opportunistic Infections Among Transgender Patients in the Clinical Setting: An All-Payer Electronic Health Record Database Study

Abstract: We performed a cross-sectional analysis of the prevalence of HIV and opportunistic infections among transgender patients in clinical care. Of 10,160 transgender patients identified, 3.9% had a diagnosis of HIV, compared to 0.32% in the non-transgender cohort (p < 0.0001). Transgender patients experience the burden of all opportunistic infection compared to non-transgender patients in this analysis, although prevalence of pneumocystis pneumonia was not significant. This cohort-based, all-payer electronic health… Show more

“…Therefore, HIV infection is one of the risk factors for HF. Research results (Niforatos et al, 2020;Dash et al, 2020;Conic et al, 2020) show that the risk of HF in HIV patients is 1.50 times that of uninfected people, which is similar to the results of this meta-analysis. Previous study (Al-Kindi et al, 2016) using case-control studies have shown that the risk of HF in HIV-infected patients is 1.64 times that of the control group, and the risk of HIV-related HF is the highest among young patients and women.…”

“…TGE individuals experience greater rates of adverse mental health concerns, such as depression, anxiety, and suicidality (Bauer et al, 2017; Borgogna et al, 2019; Mclachlan, 2019; Meyer et al, 2017), decreased psychological well-being (e.g., life satisfaction and self-esteem; Davey et al, 2014; Van Den Brink et al, 2020) along with increased severity of substance use compared to cisgender counterparts (Gonzalez et al, 2017; Shipherd et al, 2019). Moreover, poorer psychosocial and health outcomes, such as increased rates of HIV (Niforatos et al, 2020), body dissatisfaction and disordered eating (Coelho et al, 2019), and adverse childhood experiences (Schnarrs et al, 2019). Social support (e.g., from family and friends) is negatively associated with suicidal ideation (Wolford-Clevenger et al, 2018), whereas low income, sense of burdensomeness, absence of belonging and internalized minority stress are positively correlated with suicidal ideation/attempt history (Grossman et al, 2016; Klein & Golub, 2016; Seelman, 2016; Testa et al, 2017; Wolford-Clevenger et al, 2018).…”

“…HIV infection is one of the leading causes of death in the TGD population, 17 with infection prevalence in transfeminine persons estimated to be 14.1% and 3.2% in transmasculine persons. 46,47 Of the patients with newly identified HIV infections, transfeminine persons had the highest rate at 2.7% compared with cisgender men (0.9%), transmasculine persons (0.5%), and cisgender women (0.2%). 48 Radiologists should not automatically assume that all TGD patients have HIV, but they should understand that the TGD population often endures challenges in access to effective prevention and treatment.…”
