Prevalence of nutritional depletion in a large out-patient population of patients with COPD

Abstract: The prevalence of nutritional depletion was high in a large out-patient COPD population in The Netherlands, especially in female COPD patients. Depletion of FFM was associated with impaired peripheral muscle strength, independent of disease severity.

“…30 Malnutrition is prevalent in patients with COPD and may act synergistically with the depletion of specific amino acids during COPD exacerbation to play a role in the decline of circulating albumin levels. 31 Schols, et al Serum albumin level decreased with declining in pulmonary function same result is also found in our study. Katsura, et al found that low serum albumin can be a risk factor for poor outcome in COPD.…”

“…More recently, the prevalence of low BMI as well as low FFMI was found to be significantly higher in female than in male COPD patients, whereas no differences in FEV 1 , dyspnea score, and health status were observed between depleted and nondepleted COPD patients. 29 The reduction in body weight and FFM in COPD patients is mainly ascribed to a disturbed energy balance, since it has been shown that these patients may exhibit an increased daily energy expenditure compared with healthy subjects. 30 It is not yet clear whether malnutrition in people with COPD is the cause of their deterioration or just part of the progress of the disease 31 ; in any case it may be only partially reversed by nutritional supplementation.…”

“…Many studies that set Patients were not classified based on the presence of emphysema (lean look and significant weight loss) or chronic bronchitis (obese or without marked weight loss) since it is often difficult to distinguish these overlapping phenotypes. Furthermore, the small percentage of malnourished patients may be explained by the fact that only three of them were female, and COPD women have a higher prevalence of nutritional depletion than their male counterparts 3 . Nutritional and functional impairments begin early in this population.…”
