Introduction: Various systemic manifestations and complications have been observed throughout the course of Chronic Obstructive Pulmonary Disease. Out of those manifestations one of the important manifestations is cardiac involvement. The major and well-known cardiac complications of COPD are pulmonary vascular disease and its impact on right ventricular function, higher incidence of myocardial infarction and arrhythmias. These complications correlate inversely with survival. Aim: To study cardiac manifestations in COPD patients and its correlation with severity of the disease. Methodology: This observational study was conducted in the department of medicine at a tertiary care hospital in Solapur, Maharashtra. Data of 80 patients diagnosed and their cardiac status assessed on the basis of clinical findings, radiological changes and spirometry, electrocardiography and echocardiography. All patients were analyzed for cardiac involvement based on symptomatology, ECG and 2D-ECHO results. Results: Out of 80 COPD patients 6 patients were GOLD class 1 (mild), 28 in class 2 (moderate), 36 in class 3 (severe) and 10 patients belonged to GOLD class 4 (very severe). Cardiovascular complications were found in 40 patients out of 80. Of these, pulmonary hypertension was found in 45%, IHD in 30%, cor-pulmonale in 20% and arrhythmias were found in 10% of which supraventricular arrhythmias were seen in 75% and ventricular arrhythmia in25%. Conclusion: Our study concluded that patients with mild to moderate COPD may also have cardiac complications. Due to common symptomatology, they are difficult to diagnose. Hence, it is necessary at the time of initial diagnosis to carry out ECG and 2DECHO for early detection and for better management of these patients.

Background Pulmonary hypertension (PH) is a serious complication with complex pathogenesis in the natural history of chronic obstructive pulmonary disease COPD, with a progressively increasing frequency with a meanwhile decreasing in functional capacity. Purpose Assessment of the incidence, pathogenesis, peculiarities, and complications of PH in COPD in geriatric population worldwide. Methods We performed an analysis of randomized, retrospective, and prospective clinical, case-control and observational studies, published at the international level, according to the subject studied and target population. Four hundred ninety-seven full articles were identified after the search through engine Google Search and databases PubMed, Hinari, SpringerLink, and Scopus (Elsevier) according to the keywords and subsequent filters. Results Depending on various factors, like the population examined, the definition used for mPAP (mPAP> 20 mm Hg or ≥25 mm Hg), the severity of the lung disease, and the method of measuring PAP, a varied incidence of COPD patients with PH complication was discovered, namely 10–91%. PH prevalence increases with the COPD severity. The presence of PH is associated with acute exacerbations of COPD, reduced survival, and increasing expenses for healthcare programs. Mild to moderate levels of PH (mPAP 25–34 mm Hg) are relatively common in COPD and usually are associated with severe airflow obstruction or parenchymal destruction. Only a minority of patients (1–5%) have severe PH (mPAP ≥35 mm Hg). Conclusions Diagnosis of PH in COPD is difficult, especially in a mild form, and requires a clinical approach associated with a comprehensive set of investigations for confirming the etiology, evaluation of the functional and hemodynamical impairment severity, and important factors in the appropriate treatment election.

Inhaled therapies targeting prostacyclin pathway in pulmonary hypertension due to COPD: systematic review

BackgroundPulmonary hypertension due to chronic obstructive pulmonary disease (COPD) and interstitial lung disease (ILD) is classified as group 3 pulmonary hypertension. Inhaled treprostinil, a prostaglandin I2 analogue also known as prostacyclin, has recently been approved as a first drug for patients with pulmonary hypertension secondary to ILD. However, due to a lack of evidence, no therapies are currently approved for those with COPD-associated pulmonary hypertension. Thus, this systematic review aims to summarise the current evidence to assess the impact of inhaled prostaglandin I2 analogue use on the pulmonary hemodynamics, exercise function, lung function, and gas exchange in patients with pulmonary hypertension due to COPD.MethodsWe systematically searched the electronic databases of Medline, Embase, Scopus and Cochrane from inception to 1 February 2023. Studies of adult patients with a confirmed diagnosis of COPD-associated pulmonary hypertension who received inhaled drugs targeting the prostacyclin pathway were included in the systematic review. Case reports, systematic reviews, conference abstracts with no full text, non-full-text articles, non-English manuscripts and book chapters were excluded from this systematic review. A risk-of-bias assessment was carried out for the studies included in this review, using two different Cochrane risk-of-bias tools for randomised and non-randomised clinical trials.ResultsA total of four studies met our inclusion criteria and were included in this systematic review. The results of one prospective clinical trial showed an improvement in the pulmonary hemodynamics (e.g., cardiac index, cardiac output and mean pulmonary artery pressure) in response to inhaled prostacyclin use in patients with pulmonary hypertension secondary to COPD. However, the severity of dyspnoea, lung function, exercise capacity and gas exchange were not affected when inhaled prostacyclin was used for patients with COPD-related pulmonary hypertension.ConclusionThis systematic review demonstrated that although inhaled prostacyclin does not seem to improve COPD-related outcomes (e.g., lung function and exercise capacity), short-term use of inhaled prostacyclin has the potential to reduce mean pulmonary artery pressure and pulmonary vascular resistance without impairing ventilation-perfusion mismatch.
