Prevalence of Renal Insufficiency in cancer patients and implications for anticancer drug management

Abstract: BACKGROUND. The Renal Insufficiency and Cancer Medications (IRMA) study is a French national observational study. The results from this study of nearly 5000 patients demonstrated the high prevalence of renal impairment in a population of patients with solid tumors. METHODS. Every cancer patient who presented at oncology departments that participated in the study over at least 1 of 2 predefined periods during 2004 were included. Renal function was calculated using Cockcroft‐Gault and abbreviated Modification of… Show more

“…Severe comorbidity may affect prostate cancer detection by concealing symptoms, limiting potential treatments, and compete as premature causes of death. For example, liver disease, heart disease, and dementia may require adjustment of cancer treatment [13,14]. Radical prostatectomy is offered to men with localized disease and an expected life expectancy of more than 10 years [15], which may explain why no interaction was present among men receiving this treatment.…”

“…1,2 In their study, Cengiz et al reported that 188 (6.7%) organ tumors of varying pathologies were found in 2817 patients with chronic renal failure 3 . Although it is known to be a common pathology in the general population 4 , the prevalence of renal insufficiency (RI) among patients with cancer has only been recently assessed in the "Renal Insufficiency and Anticancer Medications" (IRMA) study 5 , where we found a high prevalence of renal insufficiency among 4684 solid tumor patients from 15 French cancer centers. Only 7.2% had elevated serum creatinine (SCR) but 50 to 60% had creatinine clearance (CrCl) below 90 mL/min, with the majority of them between 60 to 90, which defines a stage 2 kidney disease according to the K/DOQI-KDIGO official international definition 6,7 .…”

“…Given that this condition can be found in more than half of cancer patients, LMWH utilization in the prevention of VTE in this group of patients might be challenging and requires dose adjustment (with the exception of tinzaparin, which does not accumulate in patients with renal function impairment and can be administered without dose corrections). 28 Fondaparinux is an indirect inhibitor of factor Xa. It was demonstrated to be an option for VTE prevention in cancer patients hospitalized for acute medical illness or surgery.…”
