Background Tobacco use is associated with early, intermediate and long-term complications throughout the life course. With an influx of newer products containing nicotine, poly-tobacco use is slowly emerging as a public health concern, that is defined as existing tobacco users currently using two or more tobacco or nicotine products. While many studies have investigated single use tobacco, there is a paucity of research on regional patterns and socio-demographic factors associated with poly-tobacco use in India. Objectives To assess prevalence of poly-tobacco use and determine the socio-demographic factors associated with poly-tobacco use in India. Methods Data from the Global Adult Tobacco Survey 2 (GATS, 2016–17) was analysed, which included information on tobacco use among people aged >15 years. The pattern of current tobacco status was described using descriptive statistics. Multiple logistic regression models were estimated to determine factors associated with poly-tobacco use. Results The prevalence of poly-tobacco use in India was found to be 9.8%. Among the current tobacco users, the prevalence was 33%. Significant socio-demographic factors associated with poly-tobacco use included younger age, male gender, religion and backward caste. North-eastern region reported highest prevalence of poly-tobacco use in the country, followed by the central region. Conclusion The number of poly-tobacco users in India is considerably high and a matter of concern, more so in north east and central regions of the country. There is a need to create awareness about dangerous effects of all types of tobacco products and strengthen implementation of tobacco control policies with special focus on regions with high burden.

Individual, social and environmental determinants of smokeless tobacco and betel quid use amongst adolescents of Karachi: a school-based cross-sectional survey

BackgroundWith 600 million people using betel quid (BQ) globally, and smokeless tobacco (SLT) use being more wide-spread; the duo is an uphill public health concern in South Asian countries. SLT and/or BQ use increases the risk for morbidity and mortality from oral cancer. Because SLT and/or BQ use is initiated during adolescence, it renders this group more vulnerable; and particular attention is needed to curb SLT and/or BQ use to reduce related disease burden. We aimed to observe the differential individual, social and environmental features amongst SLT and/or BQ users to determine the key influencers of its use in adolescents.MethodsThis study was a cross-sectional survey of 2140 adolescents from secondary schools of Karachi, Pakistan. The main outcome measure was SLT and/or BQ use based on their consumption in the past 30 days. Univariate and multivariate regression binary logistic analyses were employed while reporting results in both crude form and adjusted odds ratio (after adjusting for all remaining individual, social and environmental level variables) with 95% confidence level. A p-value of < .05 was considered significant for all analyses.ResultsThe overall prevalence of SLT and/or BQ use was 42.6% (n = 912) of the total sample. The SLT and/or BQ consumer group had more males than females. A significant proportion of user (n = 558, 61.2%) was found in co-education schools. Students whose peers (OR = 6.79, 95% CI 4.67–9.87, p-value <0.001) and/or either of the parents (OR = 2.16, 95% CI 1.73–2.65, p-value <0.001) used SLT and/or BQ, alongside, adolescents who had not attended knowledge based sessions in schools regarding harmful effects of SLT and/or BQ were more likely to consume it. It’s availability with outside school hawkers increased the odds of its use by 6 times, as indicated by both univariate and multivariate models after adjusting for the remaining variables.ConclusionIn conclusion, students studying in co-education, parents and peers use, lack of knowledge based sessions on harmful health effects of SLT and/or BQ, and easy availability of the product from hawkers outside school all contribute towards enhanced risk of SLT and/or BQ use in adolescents.

Background: The prevalence of smoking cigarettes among adolescents in India is nearly one in 10, and almost half of them initiate tobacco use before turning 10 years old. Our study objective was to assess the prevalence of tobacco consumption and the sociodemographic factors associated with tobacco consumption among adolescents in a rural area of Rajasthan. Materials and Methods: This community-based, cross-sectional study was conducted for a period of 1 year from April 2021 to March 2022 in village Naila, Jaipur. All 1083 adolescents of village Naila were contacted; however, 1049 were included in the study. Multivariate logistic regression (forward likelihood model) was used to assess the strength of the association of risk factors for the consumption of tobacco. Results: A considerable proportion, that is, 22.4% (235/1049), of adolescents were tobacco consumers. Out of 235 consumers, 104 were smokers, 65 were consuming smokeless tobacco, and 66 were consuming both forms of tobacco. In multivariate analysis, age, sex, grade of participants, pocket money, family history of tobacco and alcohol consumption, maternal education, and per capita income were found to be significant predictors of tobacco consumption (P < 0.05). Conclusion: Tobacco consumption is highly common in Rajasthan’s rural adolescent population, and it begins at a very young age. Hence, corrective measures need to be initiated in an early stage of life. To reduce the impact of social norms, peer pressure, and family members who consume tobacco, the behavioral change communication activities must be strengthened.
