The necessity and importance of blood donation well known like in thalassemia, leukemia, lymphoma, patients on chemotherapy and after massive hemorrhage from trauma etc.but it’s transfusion can also lead to some major health problems like transmission of infectious diseases such as syphilis and so on. To prevent transmission of infectious diseases through blood transfusion its screening is performed everywhere. For this purpose, a research study is carried out during January 2019 to April 2019 at Mardan medical complex and Mardan teaching institute (MMC-MTI) Mardan KP, Pakistan. The specimens were collected from 625 blood donors with 619 males and 06 females. The samples were sent to the serology section of the pathology department for the diagnosis of syphilis through architect syphilis TP. The sero-prevalence of syphilis and the variation of prevalence in the previous and recent study were the objective of this research. It has found that only 06 male individuals are found to have syphilis infection while there is no such case reported in female individuals. So the overall sero-prevalence and sero-prevalence in male individuals were the same as 0.96%. The most positive cases were in the age group 38-74 years of age individuals. Which is the most sexually active age and it is reported that 1.8% of the sero-prevalence is found. The seroprevalance of syphilis in A- blood group was 5.5%, B+ patients had 1.56%, and with O+ patients it was 1.16%. From the results and analysis, the sero-prevalence of syphilis in the area and population where the study performed are at more risk of syphilis because per annual 0.30 increases is recorded by comparing the recent results with previous studies. As with the passage of time people are educated so we cannot say that it is because of the lack of awareness no also. So the opinion is that the lifestyle, social interaction of people with one another and carelessness in sexual intercourses is the main risk factors responsible for this increase in sero-prevalence of syphilis.

Background Women who are sexually active are at risk of sexually transmitted infections (STIs), which can cause serious difficulties for their reproductive health. However, despite the high global burden of STIs, most infections are preventable with education for behavioral change. The purpose of this study is to investigate the Efficacy of Mobile-Based Educational Intervention Using Instructional Systems Design in Promoting Preventive Behaviors for Sexually Transmitted Infections among Iranian Women. Methods This randomized controlled trial aimed at promoting preventive behaviors related to STIs in Iranian women with an educational intervention based on the Instructional Systems Design (ISD) in 2022. The participants in this study were recruited from a single center, specifically the Health House No. 3 located in District 11 of Tehran Municipality. Two instruments were used in the present study: a) a valid scale titled: “Four-Scale of STI Preventive Behaviors”, and b) a researcher-made Questionnaire titled: “Social perception affecting sexually transmitted infections (SOPESTI)”. These tools contain 8 demographic items and specific questions with a total of 68 five-point Likert scales. The intervention comprised three phases: a pre-test (baseline), a training program, and two follow-up assessments (4 and 12 weeks after the start of the training program). The experimental group received education through a mobile app, while the control group received no intervention. SPSS v.26 was used, with a significance level of P < 0.05. The chi-square test, Fisher’s exact test, independent t-tests, analysis of covariance (ANCOVA), and repeated measures ANOVA were used to analyze the data. Results A total of 80 women, with a mean age of 36.524 ± 6.91 (experiment group) and 34.78 ± 8.20 (control group), respectively, participated in the trial. The study revealed a statistically significant difference in the mean score for eight domains, including STIs Knowledge, STIs Vulnerability, STIs Preventive Self-efficacy, STIs Prevention intentions, STIs Perceived social exclusion, STIs Perceived cognitive barriers, STIs Perceived social support, and STIs Perceived risks in the experiment group following the intervention compared to before the intervention (p < 0.05). Conclusion The results of this study showed that a mobile-based educational intervention based on the ISD model had a significant effect on the preventive behaviors of STIs in Iranian women. These results highlight the potential benefit of mobile health in enhancing reproductive health. Trial registration ClinicalTrials.gov IRCT20200602047638N1. Syphilis, caused by Treponema pallidum, is resurging globally. Molecular typing allows for the investigation of its epidemiology. In Pakistan and other nations, T. pallidum subsp. pallidum has developed widespread macrolide resistance in the past decade. A study at the Peshawar Regional Blood Centre from June 2020–June 2021 analyzed serum samples from 32,812 blood donors in Khyber Pakhtunkhwa, Pakistan, to assess circulating T. pallidum strains and antibiotic resistance. Blood samples were initially screened for T. pallidum antibodies using a chemiluminescent microparticle immunoassay (CMIA). CMIA-reactive samples underwent polymerase chain reaction (PCR) targeted the polA, tpp47, bmp, and tp0319 genes. PCR-positive samples were further analyzed for molecular subtyping using a CDC-developed procedure and tp0548 gene examination. All PCR-positive samples were analyzed for the presence of point mutations A2058G and A2059G in 23S rRNA, as well as the G1058C mutation in 16S rRNA. These mutations are known to impart antimicrobial resistance to macrolides and doxycycline, respectively. Out of 32,812 serum samples, 272 (0.83%) were CMIA-reactive, with 46 being PCR-positive. Nine T. pallidum subtypes were identified, predominantly 14d/f. The A2058G mutation in 23S rRNA was found in 78% of cases, while G1058C in 16S rRNA and A2059G in 23S rRNA were absent. The research found donor blood useful for assessing T. pallidum molecular subtypes and antibiotic resistance, especially when chancres are not present. The prevalent subtype was 14d/f (51.85%), and the high macrolide resistance of 36 (78%) indicates caution in using macrolides for syphilis treatment in Khyber Pakhtunkhwa, Pakistan.
