Background: Diabetes mellitus (DM) is one of the greatest challenges for public health worldwide. The aim of the study was the analysis of diabetes development in participants with normoglycemia and Impaired Fasting Glucose (IFG) in 3-year and 6-year follow-up of PURE Poland cohort study. Methods: The analysis was conducted in Polish cohort enrolled into Prospective Urban and Rural Epidemiology (PURE) Study. The following study presents results of 1330 participants that have partaken both in the baseline study, in 3-year and in the 6-year follow up. The analysis of the impact of risk factors on diabetes development was performed using multivariate Cox frailty analysis. Population Attributable Risk (PAR) was computed individually for every risk factor. Results: Diabetes prevalence increased from 17.7% at baseline to 23.98% in 3-year-and 28.27% in 6-year follow-up. The risk of diabetes was higher in participants with obesity [HR = 5.7, 95%Cl 2,56-12,82], overweight [HR = 3.4, 95%Cl 1,56-7,54] and IFG [HR = 2.7, 95%Cl 1,87-3,85]. The risk of diabetes development was almost 2-fold higher in men than in women [HR = 1.826; 95%CI =1,24-2,69]. In 6 years, diabetes developed in 23.8% of participants with IFG and 7.9% of participants with normoglycemia. According to PAR, overweight and obesity accounted for 80.8%, hypertension for 67.6% and IFG for 38.3% of diabetes cases in our population. Conclusions: Our study reveals alarming increase in prevalence of diabetes during 6 years of observation. In our population, most diabetes cases can be attributed to overweight, obesity, hypertension and IFG. Findings add strong rationale to implement targeted preventive measures in population of high risk.

Elements in the human body come from contaminated food, water, and air from the living area. Bones are a marker of long-term exposure to elements and show a relationship between them. The aim of the study was to analyze the correlation between the contents of Zn, Cu, Fe, Mo, Cr, Ni, Ba, Sr, and Pb in the proximal femoral head (cancellous bone) and femoral neck (cortical bone) in rural and urban populations. The study included 96 patients who were operated on for total hip replacement (THR), acquired in a surgical procedure with atomic absorption spectrometry, and the content of Zn, Cu, Fe, Mo, Cr, Ni, Ba, Sr, and Pb was evaluated. In rural areas, significant negative correlations were observed for Mo/Cr, Mo/Cu, and Ni/Fe, and positive correlations were observed for Fe/Zn and Pb/Zn. In urban areas, a negative correlation was found for Pb/Mo. Pb and Ni increased with age only in villagers, and Zn and Sr decreased with age in urban citizens. Ba decreased with age in people from rural areas. The correlation showed variances mainly in molybdenum, nickel, and oxidative elements between rural and urban populations.

Aim of the study: To assess lung function and its relationship with smoking status and respiratory symptoms in the Prospective Urban and Rural Epidemiology (PURE) Poland Study population. Material and methods: Analysis was carried in a cohort consisting of 2036 adults enrolled in the PURE Poland Study. This paper presents the results of 1217 (773 female) subjects in whom spirometry was performed and who were assessed for smoking status. We estimated the odds ratio for spirometry parameters values (forced respiratory volume in 1 second [FEV1], forced vital capacity [FVC], and FEV/FVC) using logistic regression, adjusted for sex, age, smoking status, and respiratory symptoms in the last six months. Results: Forty eight percent of participants were never smokers, 34% were former smokers, and 17% were current smokers. Chronic obstructive pulmonary disease (COPD) was diagnosed in 1.52%, 1.45%, and 2.39% of this population, respectively, and asthma in 4.88%, 1.69%, and 2.87%, respectively. Respiratory symptoms in the last six months were more common in current smokers than never smokers and former smokers. Conversely, spirometry parameters values (FEV1, FVC, and FEV1/FVC) did not differ significantly between the groups. In the logistic regression analyses, the strongest decrease of risk factors for FEV1/FVC, FEV1, and FVC were increasing age, and for Tiffeneau index former smoking status. Among women the decrease in FVC was more pronounced than in men (OR 3.82; 95% CI 2.18-6.68). The presence of respiratory symptoms did not increase the risk of decline of spirometry parameters. Conclusions: Lung function based on spirometry parameters in the surveyed PURE Poland Study population was more dependent on increasing age and female sex than on smoking status and respiratory symptoms.
