Introduction: Urinary incontinence is a geriatric syndrome that generates strong economic and social impacts as well as emotional changes and negative consequences for the health of the elderly. Objective: The objective of this work was to identify the prevalence of urinary incontinence and its associated factors, as well as its impact on quality of life of institutionalized elderly. Materials and methods: A cross-sectional study was conducted herein, with 10 long-term institutions for the elderly of the municipality of Natal (Northeast Brazil). The Minimum Data Set was completed by caregivers to determine the presence of incontinence, and the Short Form of the International Consultation on Incontinence Questionnaire was answered by the elderly, to assess the frequency and amount of urinary leakage, the impact on quality of life, and the type of incontinence. The chi-square test and Fisher's exact test were applied in the bivariate analysis, and logistic regression was utilized for multivariate analysis. Results: The prevalence of urinary incontinence was 42.7% (95% CI: 34.8-50.8). The impact of this condition on daily life was mild in 46.5% of cases, moderate for 29.3% of cases, and severe in 24.1% of cases. The condition was associated with functional impairment (p < 0.001; RP: 4.13).

RESUMOIntrodução: São vários os fatores relacionados com a ocorrência da Incontinência Urinária (IU), entre eles doenças prevalentes na população, como o Acidente Vascular Encefálico (AVE). Objetivo: Verificar o perfil dos pacientes acometidos pela IU pós-AVE atendidos em um centro de reabilitação de alta complexidade no interior do Rio Grande do Sul. Métodos: Trata-se de um estudo transversal, no qual foram incluídos pacientes de ambos os sexos que apresentaram IU pós-AVE e excluídos aqueles que apresentavam IU devido a outras patologias associadas ao Sistema Nervoso Central (SNC) ou sistema nervoso periférico. As principais variáveis analisadas foram sexo, idade, tipo de AVE, antecedentes patológicos, presença de IU pré e pós-AVE, Índice de Massa Corporal (IMC), além da utilização do Índice de Barthel e do International Consultation on Incontinence Questionnaire -Short Form (ICIQ-SF). Resultados: Amostra composta por 21 indivíduos, com idade média de 56 ± 12,44 anos. Todos eram continentes antes do AVE e a maioria evoluiu para a incontinência. A predominância do tipo de AVE foi de etiologia isquêmica, houve um maior grau de dependentes leves na realização das atividades da vida diária, já conforme o ICIQ-SF a perda de urina causa um impacto tido como severo. Conclusão: Concluímos que houve uma ligeira predominância de mulheres que sofreram AVE e se que tornaram incontinentes, apresentando nível de dependência leve, embora o impacto que a perda urinária gera foi considerado severo, interferindo na qualidade de vida.Palavras-chave: Incontinência urinária. Predicting continence decline in institutionalized older people: A longitudinal analysis

Aims To verify the incidence of urinary continence decline and the probability of maintaining urinary continence, as well as identify the prognostic factors of urinary continence decline in institutionalized older people. Methods A 2‐year longitudinal study (with five 6‐month waves) was conducted with subjects ≥60 years old who lived in 10 nursing homes in the city of Natal‐RN (Brazil). Urinary incontinence was assessed by the Minimum Data Set version 3.0. Sociodemographic, institution‐related and health‐related variables were considered to establish the baseline. Time‐dependent variables included cognitive decline, functional decline, and incidences of falls, hospitalizations, and fractures during the period. The actuarial method, the log‐rank test, and Cox’s regression were applied as statistical methods. Results Among the cohort of 196 older adults, 105 (53.6%) individuals maintained the continence status during the period, 21 (10.7%) improved it at one or more assessments, and 76 (38.8%) subjects declined. The cumulative probability of maintaining continence status was 82.6% (confidence interval [CI], 95%: 76.5%‐87.3%), 74.7% (CI, 95%: 67.8%‐80.4%), 66.9% (CI, 95%: 59.4%‐73.2%), and 49.3% (CI, 95%: 40.1%‐57.9%) at 6, 12, 18, and 24 months, respectively. Predicting factors for continence decline were: disability (hazard ratio [HR] = 4.03; P < 0.001), functional decline (HR = 3.02; P = 0.001) and potentially inappropriate medication (HR = 1.84; P = 0.008). Conclusions The incidence of continence decline and the cumulative probability of maintaining continence status in institutionalized older adults was approximately 39% and 49%, respectively, at the 2‐year follow‐up. Disability and potentially inappropriate drugs at baseline and functional decline across the period predicted continence decline in this cohort.

Abstract: It is concluded that UI is a health issue that affects more than half of the institutionalized elderly, and is associated with white race, physical inactivity, stroke, and other geriatric syndromes such as immobility and cognitive disability.
