Prevention and treatment of anticipatory chemotherapy‐induced nausea and vomiting in pediatric cancer patients and hematopoietic stem cell recipients: Clinical practice guideline update

Abstract: This 2021 clinical practice guideline update provides recommendations for preventing anticipatory chemotherapy‐induced nausea and vomiting (CINV) in pediatric patients. Recommendations are based on systematic reviews that identified (1) if a history of acute or delayed CINV is a risk factor for anticipatory CINV, and (2) interventions for anticipatory CINV prevention and treatment. A strong recommendation to optimize acute and delayed CINV control in order to prevent anticipatory CINV is made. Conditional reco… Show more

“…Aromatherapy is an effective method to manage POV (26). Systematic desensitization, hypnosis, relaxation techniques, and lorazepam can be considered for the prevention of CINV (12). Numerous therapies are free or cheap, making them a convenient adjunct to medication.…”

“…Due to the differences in chemotherapy doses and regimens between children and adults, the risk factors of CINV in children may also be different from adults (11). The 2021 clinical practice guideline is strongly recommended to optimize acute and delayed CINV control to prevent anticipatory CINV (12). In the first 24 hours, the complete control rate of highly emetic chemotherapy (HEC) in pediatric patients was 37% and that of MEC in such patients was 24% (5).…”

“…Lorazepam can be considered to prevent anticipatory CINV, but the evidence guiding this recommendation is weak and the optimal paediatric dose is not established. 6 Some data indicate that cannabinoids are of therapeutic value in the control of CINV in children, but no paediatric trials compare the efficacy and safety of cannabinoids with modern first-line antiemetic therapy. 10…”

“…POGO has developed guidelines for breakthrough, refractory, and anticipatory CINV in children, but the recommendations within these guidelines are generally weak. 5,6 No paediatric guideline includes antiemetic strategies for delayed CINV or multiday chemotherapy regimens.…”
