Purpose To report morphological patterns of osteoporotic vertebral compression fractures (OVCFs) presenting for surgery. To describe surgical options based on fracture pattern. To evaluate clinical and radiological outcome. Methods Forty consecutively operated OVCFs nonunion patients were retrospectively studied. We define four patterns of OVCFs that needed surgical intervention. Group 1 mini open vertebroplasty (N = 10) no neurologic deficits and kyphotic deformity, but with intravertebral instability and significant radiological spinal canal compromise. Group 2 with neurologic deficits (N = 24) (2A)-transpedicular decompression (TPD) with instrumentation (N = 14). Fracture morphology similar to (1) and localized kyphosis \30°(2B)-pedicle subtraction osteotomy (PSO) with instrumentation (N = 10). Fracture morphology similar to (1) and local kyphosis [30°. Group 3 posterolateral decompression with interbody reconstruction (N = 06) endplate(s) destroyed, with instability at discovertebral junction, with neurologic deficit. Average follow-up was 34 months. VAS, ODI and Cobb angle were recorded at 3, 6, 12 months and yearly. Results There was significant improvement in the clinical (VAS and ODI) scores and radiologic outcome in each group at last follow-up. 30 patients out of 40, had neurologic deficits (Frankel's grade C = 16, Frankel's grade D = 14). The motor power gradually improved to Frankel's grade E. Average duration of surgery was 97 min. Average blood loss was 610 ml. Conclusion Different surgical techniques were used to suit different fracture patterns, with good clinical and radiological results. This could be a step forward in devising an algorithm to surgical treatment of OVCF nonunions.

ResumoOBJETIVO: Avaliar o impacto da prática de atividade física na qualidade de vida de mulheres de meia idade. MÉTODOS: Estudo de base populacional e corte transversal, que incluiu uma amostra estratificada de 370 mulheres de meia idade entre 40 a 65 anos, recrutadas a partir de uma população de 20.801 mulheres atendidas no período de um ano nas redes básicas de saúde, inseridas nos quatro distritos (Norte, Sul, Leste e Oeste) que compõem o sistema de saúde da cidade de Natal, Rio Grande do Norte, de junho a setembro de 2011. O cálculo da amostra teve por base um nível de confiança de 95%, com poder do teste de 80%, erro de estimativa de 5% e considerou-se a proporção de pacientes classificadas com qualidade de vida adequada (indicador ≥26) da amostra piloto. Os dados foram coletados enquanto as mulheres aguardavam na sala de espera para a consulta de rotina. Para avaliar a qualidade de vida geral, utilizou-se a versão abreviada do WHOQOL (WHOQOL-Bref-WHO Quality of Life -BREF), e sua relação com os sintomas do climatério foi avaliada por meio do Menopause Rating Scale (MRS). O nível de atividade física foi avaliado pelo questionário International Physical Activity Questionnaire (IPAQ), versão curta, semana usual. Para obter-se a classificação dos níveis de atividade física, utilizaram-se três categorias: sedentária, moderadamente ativa e muito ativa. A análise estatística foi realizada utilizando o programa estatístico Minitab, versão 16. RESULTADOS: A média de idade das mulheres foi de 49,8 anos (±8.1), foram predominantemente caucasianas (72,7%), casadas (61,6%), não fumantes (93,5%) e com o Ensino Médio completo (47,8%). Considerando os domínios presentes no WHOQOL-Bref para avaliar qualidade de vida, os escores foram significativamente diferentes entre os grupos de mulheres sedentárias, moderadamente ativas e muito ativas (p<0,01). Em relação à atividade física e aos sintomas do climatério, foram observadas diferenças significativas para todos os domínios: psicológico (p<0,01), somático-vegetativo (p<0,01) e urogenital (p<0,01). CONCLUSÃO: A prática de atividade física melhora significativamente a qualidade de vida das mulheres de meia idade. Purpose In the present study, the fabrication of novel p(N-isopropylacrylamide-co-butyl methylacrylate) (PIB) nanogels was combined with boron-containing mesoporous bioactive glass (B-MBG) scaffolds in order to improve the mechanical properties of PIB nanogels alone. Scaffolds were tested for mechanical strength and the ability to promote new bone formation in vivo. Patients and methods To evaluate the potential of each scaffold in bone regeneration, ovariectomized rats were chosen as a study model to determine the ability of PIB nanogels to stimulate bone formation in a complicated anatomical bone defect. PIB nanogels and PIB nanogels/B-MBG composites were respectively implanted into ovariectomized rats with critical-sized femur defects following treatment periods of 2, 4, and 8 weeks post-implantation. Results Results from the present study demonstrate that PIB nanogels/B-MBG composites showed greater improvement in mechanical strength when compared to PIB nanogels alone. In vivo, hematoxylin and eosin staining revealed significantly more newly formed bone in defects containing PIB nanogels/B-MBG composite scaffolds when compared to PIB nanogels alone. Tartrate-resistant acid phosphatase-positive staining demonstrated that both scaffolds were degraded over time and bone remodeling occurred in the surrounding bone defect as early as 4 weeks post-implantation. Conclusion The results from the present study indicate that PIB nanogels are a potential bone tissue engineering biomaterial able to treat defects of irregular shapes and deformities as an injectable, thermoresponsive, biocompatible hydrogel which undergoes rapid thermal gelation once body temperature is reached. Furthermore, its combination with B-MBG scaffolds improves the mechanical properties and ability to promote new bone formation when compared to PIB nanogels alone.
