“…The antireflux effects of solidifying enteral nutrients have been investigated in patients with PEG or healthy adult volunteers [4,7,8], but in these studies only a small number of subjects is included, and the influence of pre-existing GERD is not examined. The usefulness of fiberoptic spectrophotometer Bilitec to qualify GER has been demonstrated [9,10].…”

“…Percutaneous endoscopic gastrostomy (PEG), which was first described by Ponsky and coworkers in 1980 [3], has become the modality of choice for providing enteral access to patients who require long-term enteral nutrition. In patients with PEG, GER was documented in 58.8% of the subjects [4] by computed tomography of the esophagus following administration of liquid nutrients containing a watersoluble contrast material. In a large study including a total of 160,983 Japanese who underwent a gastric check up, the prevalence of symptoms strongly suggestive of GERD was rather high, at about 20% of this population [5].…”

“…These complications have been reported to be improved with semi-solid formulas compared with liquid formulas [11][12][13][14]. Most semi-solid formulas have been administrated as a bolus using a syringe by hands through the current connectors.…”

“…Aspiration pneumonia is a serious adverse event in enteral tube feeding [9,10]. Semi-solid enteral formulas prepared by the addition of pectin or agar to the liquid formula had been reported to reduce the risk of gastroesophageal reflux (GER) relative to that of the liquid formula in the patients fed via PEG tubes [11][12][13]. Semi-solid formulas are becoming widely used in Japan to reduce the risk of GER and aspiration pneumonia in such patients.…”

“…The patients with scores ^ 2 appear to be at very low risk for aspiration pneumonia, and those with scores 6 3 should be carefully followed up. It should be considered that a semi-recumbent position [10] , jejunal tube feeding via the PEG tube [11] and conversion to half-solid nutrients [12] could prevent GER and aspiration pneumonia in patients with PEG.…”
