Previous synovectomy or epiphyseal stapling and the influence on knee replacement in juvenile chronic arthritis

Abstract: Seventy-seven anatomically graduated components (AGC) total knee arthroplasties (TKA) were performed on 52 patients with juvenile chronic arthritis. According to the nature of previous surgery on the knee, the patients were subdivided into three groups. The mean age at onset of disease in 23 patients with previous synovectomy of the knee was 11 (1.5-16) years, the mean age at the time of synovectomy was 20 (4-42) and the mean age when arthroplasty was performed was 31 (18-45) years. In nine patients with previ… Show more

“…Generally, the inflamed and thickened synovial tissue is preferred to be resected by most of the surgeons for inflammatory arthritis [12, 19]. However, in primary osteoarthritis, the advantages and disadvantages of combining the total capsulosynovectomy with total knee arthroplasty have not been determined.…”

“…Lybäck et al [ 16 ] conducted a case-control study of 77 patients with JIA who underwent total knee arthroplasty. Patients with prior synovectomy had shorter mean time to arthroplasty compared to patients with no synovectomy.…”

“…Publication dates ranged from 1969 to 2013. One article was a randomized trial, [10] 1 was a case-control study, [16] and all others were case series. [9,11,[13][14][15][17][18][19][20][21][22][23] Of the 11 articles, 13 were level-IV studies, and 1 was a level-II study (Table 1).…”

“…However, in the knee, release requires both soft tissue and bony resection with particular attention to the posterior neurovascular structures. Previous synovectomy or epiphyseal stapling can influence surgical outcomes [2]. Soft tissue management in JRA patients is of paramount importance in determining a successful surgical outcome of total joint replacement.…”
