Primary adenoid cystic carcinoma of lung: A case report and review of the literature

Abstract: Primary adenoid cystic carcinoma of lung is an unusual thoracic neoplasm, which is considered as a slow-growing low-grade malignancy. The primary treatment for this tumor is surgery. The role of radiotherapy, chemotherapy, and targeted agents is less well defined. Here we report a case of inoperable adenoid cystic carcinoma of the lung in a 42-year-old male, presenting with an unusual aggressive behavior. The patient received radiotherapy and oral tyrosine kinase inhibitor imatinib with effective palliation.

“…Nearly 8% of young patients in our study were uniquely diagnosed with pulmonary ACC, a histology that is far more frequently identified within tumours of the head and neck [ 27 ]. Salivary gland-type tumours are rarely diagnosed as lung cancers, and historically, mucoepidermoid carcinoma (MEC), fol lowed by ACC and epithelial–myoepithelial carcinoma, is most common [ 28 , 29 ].…”

“…Taking these data into consideration, a novel therapeutic strategy must be sought for advanced ACCL. The use of imatinib has been reported for one advanced case and achieved a satisfactory effect ( 24 ).…”

“…The solid histological pattern has been associated with a more aggressive clinical course and early distant metastases, in contrast to the cribriform type which shows a more benign behaviour. [8] The mainstay of therapy is surgery. Our patient probably had post obstructive pneumonia and an empyema as a result of the tumour in the left upper lobe.…”
