Primary Cardiac Leiomyoma of the Ventricular Septum: A Rare Form of Pediatric Intracardiac Tumor

Abstract: A case of ventricular septal tumor diagnosed in 7-month-old boy is reported. An echocardiogram was performed for investigation of a heart murmur in an otherwise healthy infant. He remained asymptomatic, and the tumor had a very slow growth. When the boy was 9 years old, partial surgical excision was performed for right ventricular outflow tract obstruction. Histology showed a primary cardiac leiomyoma. To the authors’ knowledge, a primary cardiac leiomyoma of the ventricular septum has not been reported previo… Show more

“…Two cases of cardiac leiomyomas in boys demonstrated the existence of primary forms of cardiac leiomyoma [5,6]. The pathologic finding of a direct connection of the tumor with the tunica media of the intramyocardial vessel in our patient is similar to the report mentioned earlier [6].…”

“…There are only 3 female and 2 male cases reported in the English literature. The origins of the first 2 female cases [2][3] were identified as metastases, and the third female case [4] and 2 male cases [5][6] were identified as primary lesions.…”

“…Primary leiomyomas of the heart are extremely rare. Those which occur are a result of cardiac extensions of intravenous leiomyomatosis through the inferior vena cava (Melo et al 2012). Others result from metastatic benign leiomyoma (MBL) and also from venous seeding following hysterectomy in case of women patients (Consamus et al 2014).…”

“…Primary cardiac leiomyoma has been described in three cases. In these cases the neoplasia originated from the right ventricle wall, creating a right ventricular outflow obstruction [6,7], or extensively involving the ventricular septum [8]. In our patient, symptoms (dyspnea, palpitation, and syncope) were related to severe right outflow obstruction and tricuspid regurgitation caused by the mass.…”
