Primary Craniovertebral Anomalies and the Hindbrain Herniation Syndrome (Chiari I): Data Base Analysis

Abstract: This prospective study analyzes 100 patients with Chiari malformation and primary craniovertebral junction (CVJ) anomalies (3–66 years). Neurodiagnostic investigations employed tomography, gas myelography, CT and CT myelography, and MRI. Factors considered were reducibility, mode of encroachment, cerebrospinal fluid (CSF) dynamics and syringohydromyelia. Sixty-six patients with irreducible pathology underwent ventral or ventrolateral decompression and dorsal stabilization. Dorsal occipitocervical fixation was … Show more

“…The incidence of such anomalies was low compared with other series 43,46 in which these abnormalities appear on average to be present in every fourth patient. The reason for this discrepancy is most likely referral bias.…”

“…There were no clear differences in the incidence of major symptoms between this series and other published reports. 2,5,9,13,16,20,31,34,35,38,43,46,51,54,55,57,59,65,68,70 Not surprisingly, there was a major difference in the time from onset of symptoms to surgery (11 months in this series), compared with pre-MR imaging series. 54,57,59 During the last decade this interval between onset of symptoms and treatment has been similar in other reports.…”

“…Osseoligamentous CVJ pathology is often associated with Chiari I malformation. 66 Although reduction 25,66 or ventral decompression may treat the osseoligamentous pathology, 60 a dorsal decompression may still be warranted in each case to treat the Chiari malformation or to ensure complete cervicomedullary decompression. The algorithm is a general guide for CVJ pathology and should be used as such when determining the appropriate treatment strategy for each patient.…”

“…We have pointed out before that atlas assimilation and segmentation failures of the upper cervical spine may contribute to ligamentous laxity and proliferation of granulation tissue at the CCJ. 17 Such instability may eventually lead to basilar invagination and irreducibility, and thus a stabilization procedure is indicated.…”
