Primary hemiarthroplasty for treatment of unstable pertrochanteric femoral fractures (AO/OTA Type 31 A2.3) in elderly osteoporotic patients

Abstract: Introduction: The aim of this study was to prospectively analyze the role of primary hemiarthroplasty in unstable osteoporotic pertrochanteric fractures (AO/OTA Type 31 A2.3), with emphasis given to postoperative Functional Independent Measure (FIM) and Harris Hip Score (HHS). Methods: Fifty-six consecutive patients (average age 78.25 ± 5.45), out of which 24 males (79.29 ± 4.99) and 32 females (77.47 ± 5.72), with unstable pertrochanteric femoral fractures, operated with primary hemiarthroplasty procedure fro… Show more

“…24 Another study reported that THA was associated with better functional outcomes and lower reoperation rates than HA for the treatment of displaced femoral neck fractures in the elderly individuals, similar to findings by Liao et al 21 However, more current data showed no significant differences in the complication rates between HA and THA. 10,21,25,26 Moreover, the literature shows lower risk of reoperation after THA compared to HA and better functional outcomes for patients following THA than that with HA 6,10,22,26 as observed in this study.…”

“…Treatment using cephalomedullary nailing of unstable IT fractures occurring in bones with poor quality shows high failure rates. 21 However, we did not observe a difference in reoperation rates between the two groups. The most important finding of the present study was that patients treated with HA showed superior functional outcomes, whereas those treated with PFN showed better health status scores at the 3-month follow-up.…”

“…Han recomendado el reemplazo protésico para permitir la carga inicial de peso en pacientes adultos mayores con una fractura intertrocantérea inestable. Por su parte, Choi et al (2010) han demostrado resultados favorables en el seguimiento a corto plazo después de realizar artroplastía de cadera para fracturas intertrocantéricas inestables (Sidhu et al, 2010;Ukaj et al, 2017).…”
