Primary mucinous carcinoma of the skin: a case of metastasis after 10 years of disease-free interval

Abstract: Primary mucinous carcinoma of the skin (MCS) is a rare neoplasm. Clinically, it has a high local recurrence rate, but it is known to be a slow-growing benign tumor with a rare incidence of distant metastases. We present a case of primary MCS on the jaw that underwent tumor resection twice and was disease-free for 10 years after the second surgery. The patient had no evidence of local recurrence and distant metastasis until his 11th year follow-up. At that time, he was diagnosed with lung and bone metastasis an… Show more

“…Local recurrence is frequent (20–30%) while lymph node and distant metastases are uncommon (11% and 3–11%, respectively) . Only two cases with fatal outcome because of metastatic spread have been reported . The natural history of our case suggests that the neuroendocrine component, identified in every lymph node metastasis and recurrence, was mainly responsible for the aggressiveness of the tumor.…”

“…3,13,22,23 Only two cases with fatal outcome because of metastatic spread have been reported. 24,25 The natural history of our case suggests that the neuroendocrine component, identified in every lymph node metastasis and recurrence, was mainly responsible for the aggressiveness of the tumor. This differs from what has been reported in breast mucinous carcinomas, in which neuroendocrine differentiation is associated with a better prognosis because of more favorable microscopic and immunohistochemical parameters, and moreover it affects older patients.…”

“…10 One case documented six local recurrences without metastasis over 30 years. 11 The mainstay of treatment is surgical excision, but remission has been achieved with the aromatase inhibitor Anastrozole in a metastatic estrogen receptor positive case.…”

“…Nos patients ont bénéficié d'une exérèse large et les limites chirurgicales périphériques étaient saines. Le taux de récidive locale est de près de 50% [ 12 ] et les métastases à distance surviennent dans 2,7% [ 4 ]. Notre patient présente une forme classique de carcinome mucineux primitif cutané, traité avec succès par une exérèse élargie.…”
