Primary pancreatic hydatid cyst: a case report and literature review

Abstract: Background Hydatid cysts are parasitic zoonoses that often occur in the liver. Pancreatic hydatid cysts are very rare and are usually misdiagnosed as pancreatic cystadenomas. At present, surgical resection combined with albendazole administration is the standard treatment for pancreatic hydatid cysts. However, making accurate preoperative diagnoses and avoiding intraoperative cystic rupture are challenges for surgeons. Case presentation A 28-year-o… Show more

“…Most of the aspects of primary PHC, including its natural history, diagnosis, and management, are not clear so far [ 8 ]. The pancreas is thought to be involved primarily through hematogenous dissemination [ 3 ]. Pancreatic hydatid involvement is solitary in a majority of cases [ 1 , 3 ].…”

“…Primary involvement of the pancreas by the hydatid disease is very rare. It accounts for < 2% of total systemic echinococcosis cases [ 2 ] with only 33 cases reported in literature since 2011 [ 3 ]. Pancreas being a rare site of involvement, primary isolated pancreatic hydatid cyst (PHC) may pose diagnostic and management challenges as it may mimic common pathologies like pseudocyst [ 4 , 5 ].…”

“…Hydatidosis is a zoonosis caused by Echinococcus granulosus. It is most commonly located in the liver (5075%) [8] . The main complications of the liver hydatid cyst are infection and rupture, especially into the biliary tract, the peritoneum and thorax [9] .…”

“…The conservative techniques are used in 75% of cases [9] . It seems adapted to an infected cyst in an emergency context as it is rapid and does not require a particular experience in liver surgery [8] . Radical surgery may be carried out if surgeon consider it technically feasible, secure and with no added risk [10] .…”

“…The mainstay of therapy for a PHC is the radical resection of the PHC and albendazole administration. Surgical procedures depend on the location of the HC in the pancreas [ 3 ]. In this case report, we present the case of a PHC as an unusual cause of abdominal pain that was misdiagnosed as a peritoneal HC on endoscopic ultrasound (EUS).…”
