Primary pulmonary choriocarcinoma in a male

Abstract: SummaryThe authors report a case of a 48-year-old male smoker who presented with respiratory distress and constitutional symptoms along with gynaecomastia and multiple pulmonary nodules. Based on normal physical and ultrasound findings of the testes, presence of markedly elevated serum β human chorionic gonadotrophin (HCG) and biopsy findings of the lung a diagnosis of primary pulmonary choriocarcinoma was made. “…Because most patients are diagnosed at an advanced stage of tumor metastasis, certain authors believe that all patients should receive chemotherapy. Adjuvant chemotherapy with two to four cycles of bleomycin, etoposide, and cisplatin (ie, PEB protocol) has been recommended for patients with ovarian choriocarcinomas 4,16. Most of the 13 PUMCH patients received chemotherapy with actinomycin D, etoposide, methotrexate, cyclophosphamide, and vincristine (ie, EMA/CO protocol); the median number of cycles was seven (range, 3–27 cycles).…”

“…In men, the most common primary site is the testis. 1 When the primary tumor is extragonadal, it can occur in the mediastinum, 2 retroperitoneum, 3 lung, 4 brain, 5 or digestive tract. 6 Because its clinical symptoms are atypical, early diagnosis is difficult.…”

“…However, the best time for surgery is always missed when carcinoma is identified due to the high degree of malignancy and early bleeding. Thereafter, the only option left is conservative treatment such as chemotherapy, which includes treatment with drugs such as etoposide, cisplatinum, methotrexate, actinomycin D, bleomycin, 5-fluorouracil, VP-16, cyclophosphamide and vincristine (10)(11)(12). However, these drugs are only partially effective, as the majority of patients succumb within a short follow-up time.…”

“…The present patient serves as a typical example to this effect, since multi-organ metastasis was observed, the symptoms were only displayed for 4 days prior to admission and the patient succumbed after the fourth day of admission. Similarly, based on literature reviews, the majority of patients succumb to organ bleeding, respiratory failure, hemorrhagic shock, disseminated intravascular coagulation and abdominal compartment syndrome (11). The occurrence of liver or brain metastases is associated with a worse prognosis (14).…”

“…However, some cases have been reported to emerge in the lung, gastrointestinal tract, brain, and liver. In the literature, only 29 cases of primary choriocarcinoma in lung have been reported in males, including 25 cases of choriocarcinoma, [1,3–11,13–19,21–22,2426] 3 cases of choriocarcinoma combined with adenocarcinoma, [12,20,23] and 1 case of mediastinopulmonary choriocarcinoma, [2] which is even rarer and has a very poor prognosis compared to its counterpart in the midline structures according to the literature. Primary pulmonary choriocarcinoma (PPC) in males is easy to misdiagnose or delay the diagnosis; therefore, a potentially curative chemotherapy or surgery may also be delayed.…”
