Primary pulmonary Hodgkin's lymphoma and a review of the literature since 2006

Abstract: A 28-year-old woman of Laotian origin presented with dry cough, fevers, night sweats and weight loss over the preceding 6 weeks. Radiological investigations revealed large complex cavitary lesions in the middle and lower lobes of the right lung, with no mediastinal lymph node enlargement. Bronchoscopic investigations did not yield a diagnosis. CT-guided fine-needle aspiration raised the possibility of Langerhans cell histiocytosis. Open lung biopsy initially supported this, but eventually a microscopic and imm… Show more

“…The criteria for the diagnosis of PPDLBCL include the following: pathological and immunohistochemical features of DLBCL, a primary lesion restricted to the lung (parenchyma and/or bronchi) with or without minimal hilar lymph node involvement, and clinical, radiological, and pathological exclusion of the disease at distant sites. 4 , 5 Our case meets these criteria.…”

“…PPHL occurs predominantly in women with a bimodal age distribution, with the first incidence peak occurring in the third decade, and the second being in the sixth/seventh decade 3. There are three main diagnostic criteria for PPHL: (1) the tumour needs to be confined within the lung tissue with or without involvement of the hilar or mediastinal lymph nodes, (2) pathological results should comply with Hodgkin’s lymphoma and (3) exclusion of other pathological conditions outside the lungs 2. Our patient had skeletal metastasis at the time of diagnosis; hence, the disease cannot be characterised as PPHL.…”

“…127 There are very limited data to evaluate survival specific to primary pulmonary lymphoma, and larger series suggest similar outcomes to other types of NHL. 128 Overall survival rates from the ART era in HIV-infected patients with NHL range from 56 to 67%. 124,129…”
