Primary Racemose Hemangioma of the Bronchial Artery.

Abstract: A 44-year-old womanwas admitted to our hospital for repeated hemoptysis. Selective angiography of the bilateral bronchial arteries showed them to be enlarged and convoluted, and revealed shunts between the bronchial and pulmonary arteries. The patient was diagnosed as having racemose hemangiomaof the bronchial artery. Although embolization of the bronchial artery with gelatin sponges was performed, hemoptysis emerged again about one year later. Finally, segmentectomy of the right lower lung was performed and p… Show more

“…19,20 Racemose hemangioma of the bronchial artery with a bronchial-pulmonary fistula is associated with a high recurrence rate. 3,[21][22][23][24][25] In the current case, a residual shunt from collateral arteries, such as the upper bronchial artery and sub-phrenic artery, was detected by postoperative CT. However, the dilated peripheral pulmonary artery was thrombosed.…”

“…Bronchial angiography showed a tumor stain, so the tumor was thought to be a type of neoplasm receiving blood flow from the bronchial artery, thus excluding racemose hemangioma, which usually shows enlarged and convoluted vessels and shunts between the bronchial arteries and pulmonary veins. 16) Biopsy and immunohistochemistry are required for a confirmed diagnosis. 17) From the findings of H-E staining, the following differential diagnoses were considered; spindle cell carcinoma, liposarcoma, inflammatory myofibroblastic tumor, and Kaposi's sarcoma.…”

“…6 Transcatheter intervention is a less invasive technique, and the cure rate for pulmonary AVMs has been reported to be 85%-98%. 7,8 However, reperfusion of the pulmonary AVM due to recanalization or growth of an accessory vessel is an acknowledged problem.…”

“…Regarding bronchial AVMs, some cases of recurrent hemoptysis due to recanalization after embolotherapy have been reported as well. 6 In addition, Brillet et al described a higher incidence of pulmonary infarction after embolotherapy in patients with pulmonary AVM fed by abnormally enlarged systemic arteries. 9 In the current case, we think that these facts support the validity of our choice of surgical treatment.…”
