Primary renal lymphoma: an unusual finding following radical nephrectomy

Abstract: Secondary kidney involvement by disseminated non-Hodgkin’s lymphoma (NHL) is quite common and is estimated to approach 30 – 60% in NHL patients. However, primary renal lymphoma is exceedingly rare and estimated to make up less than 1% of all kidney masses. We report a case of primary renal NHL presenting with profound hypercalcemia and renal failure recalcitrant to medical management, ultimately treated with urgent radical nephrectomy. To our knowledge, this is the first report of primary renal lymphoma presen… Show more

“…Chronic inflammation invades lymphoid cells which undergo oncogenic transformation in situ . Reports of PRL arising in patients with chronic pyelonephritis support this theory ; however, PRL is known to occur in patients without any primary kidney disease . Association of PRL with other chronic inflammatory and infectious diseases such as Sjogren's disease, systemic lupus erythematosus, and Epstein‐Barr virus infections has also been reported .…”

“…3,12 Reports of PRL arising in patients with chronic pyelonephritis support this theory 3 ; however, PRL is known to occur in patients without any primary kidney disease. 7 Association of PRL with other chronic inflammatory and infectious diseases such as Sjogren's disease, systemic lupus erythematosus, and Epstein-Barr virus infections has also been reported. 3 Another theory states that PRL originates in the lymphatics surrounding the renal capsule and progress to infiltrate the kidney manifesting as solitary or multiple focal masses, large infiltrations in unilateral kidney or diffuse bilateral infiltrations.…”

“…We thoroughly reviewed all 54 cases of PRL reported in the literature since 2002 (Table 1). 4,749 DLBCL was found to be the most common pathological type, and more male than female patients (64.8%) were reported. Additionally, PRL was generally located in the bilateral kidneys in younger patients (<30 years old) and in a unilateral kidney in older patients, indicating that the site of PRL is age-related.…”

“…Lacking in specific symptoms and laboratory indexes makes PRL difficult to diagnose before surgery. [ 21 , 22 ] To date, the diagnostic criteria of PRL made by Stallone in 2000 are still widely used, [ 1 ] the criteria are as follows: lymphomatous renal infiltration, nonobstructive uni- or bilateral kidney enlargement, and no extra localization at diagnosis. The patient in our case fulfilled all 3 criteria.…”
