Primary Torsion of the Greater Omentum: Report of a Case

Abstract: Primary or idiopathic torsion of the greater omentum is an uncommon cause of acute abdominal pain, often mimicking other acute abdominal conditions. The diagnosis is usually made at laparotomy, with the presence of free serosanguinous fluid in the absence of any other intra-abdominal pathology being suggestive of this condition. Resection of the infarcted segment is the treatment of choice, offering rapid recovery and reducing the possibility of adhesion formation. We report a case of primary omental torsion a… Show more

“…The precipitating factors include sudden increase in intra-abdominal pressure following a heavy meal or exertion, change in body position, coughing or sneezing, and occupational use of vibrating tools. 8,9 Obesity has also been implemented as a risk factor. 1,10 Secondary torsion is more common and a result of underlying abdominal pathology (e.g.…”

“…One end is free and other is fixed in the primary, whereas the free end is attached either to adhesion or some pathological condition in the secondary type [6]. There are several predisposing and precipitating factors for primary omental torsion as anatomical variation of omentum including tongue-like projection from free edge of omentum, bifid omentum, accessory omentum, obesity associated with irregular distribution of fat within the omentum and narrow omental pedicle [7,9].…”

“…Omentum torsiyonunun daha çok sağ tarafta görülmesi; akut apandisit, akut kolesistit, çekal diverikülit gibi hastalıklardan ayırıcı tanısının zorlaşmasına sebep olmaktadır (4,5). Torsiyonun sağ tarafta daha fazla görülmesinin sebebi omentumun sağ tarafta sola göre daha uzun ve geniş olması olarak düşünülmektedir (6). Omental torsiyon gelişimi iki şekilde olabilir.…”
