Prism intervention helped sustainability of effects and ADL performances in chronic hemispatial neglect: A follow-up study

Abstract: To investigate the sustainability of effect and the changes in basic and instrumental activities of daily living (ADL and IADL) after long-term prism intervention, we carried out a follow-up assessment on five subjects with hemispatial neglect in the chronic stage. Method: At 2-3.5 years after the end of prism intervention, we measured six parameters: 1) eye movement of the neglect side when watching three different video clips, 2) center of gravity as an index of subjective internal midline bias, 3) star and … Show more

“…Other authors have proposed the presence of ocular motor changes, that could influence spatial representations. Such hypothesis has been then confirmed by successive studies, which have highlighted an increase in ocular movements toward the neglected side after a PA training [23, 28]. With regard to the present study, even though we have not found significant differences between scores in representational drawing subtest across sessions, the only patient with representational neglect showed a complete and long-lasting recovery after PA training.…”

“…A recent work in this field of study has shown that amelioration of neglect consequent to PA training lasts up to two years after the end of the treatment in patients in the chronic stage of the disease [28]. …”

“…Rehabilitation studies reporting negative findings in neglect patients employed treatments with shorter duration (Nys et al, 2008, 4 days), or weaker displacing lenses (Turton et al, 2010, 6° lenses). Importantly, long-term training has shown positive impact on functional abilities of everyday life, as assessed by Activities of Daily Living (ADL) Scales: the FIM™ (Tesio et al, 2002) scale (Fortis et al, 2010; Mizuno et al, 2011); the Barthel Index (Mahoney and Barthel, 1965), and Lawton's IADL scale (Shiraishi et al, 2008, 2010). In sum, it is preferable to use an adaptation procedure more appreciated by patients, given the length (at least 2 weeks) of the treatment.…”

“…Specifically, with a mortality of 48.2 persons per 100,000 population in 2014, stroke was the 3rd most common cause of death1 ) . In stroke patients, impaired visual perception not only reduces the overall ability to perform activities of daily living (ADLs), but also causes difficulties in differentiating between objects or letters2 ) . Because of the negative effects of unilateral neglect and decreased spatial awareness on overall ADLs, and particularly in dressing, hygiene, movement, independent living, and learning new behaviors, impaired visual perception has a major impact on rehabilitation3 ) .…”
