Self-care of patients with chronic noncommunicable diseases is an essential component of contemporary healthcare. The purpose of this paper is to present a novel self-care process model and place it in the broader context of professional care. The extended Event-driven Process Chain approach to process modelling was used, focusing on a detailed overview of sequences of events, connections and activities and other elements/building blocks. A self-care process model was designed. The model is divided into two parts. The first part represents the self-care process when patients are able to manage their symptoms and be independent. The second part includes the process when patients are unable to perform self-care and/or need professional support. By identifying the essential elements of this process and incorporating them into the patients' care process, we can ensure that professional support for self-care creates a dynamic balance in the patients’ ecosystems. Patients with chronic noncommunicable diseases need to make timely decisions about individual aspects of their health and seek professional help. In this way, an optimal level of health and well-being of patients can be achieved. Focusing on the patients’ self-care process could also reduce treatment costs and improve the quality of life of patients. The novel designed model of the process of self-care, with all its essential elements, can be supported by digital technology, especially in the decision-making process and needs to become an important part of healthcare and long-term care systems.

Currently, the number of patients with neurological diseases is increasing, especially those older than 65 suffering from Alzheimer’s disease. This development increases the emphasis on understanding and mapping treatment and care processes, not only for the elderly. Service providers (of both treatment and care) are under general pressure to decrease charges and maintain or improve existing levels of care. This situation is significantly influenced by a comprehensive knowledge of the whole process and its values. This publication therefore aims to describe the fundamental procedural aspects of caring for patients with Alzheimer’s disease, using Business Process Model and Notation (BPMN). It also aims to show the possibilities of using BPMN in the description of treatment and care. Modeling of the business process is more frequently being applied not only by businesses but also by scientists involved in process models. It is used to model medical topics, with approximately 10% of its publications only, and most of these publications deal only with clinical pathways, not with overall treatment and care processes. However, the BPMN model allows the whole process of medical and nonmedical care for patients with Alzheimer’s disease to be described, including the decomposition of partial activities into individual threads and sub-processes or atomic tasks. This paper presents the BPMN modeling and mapping of the specific care path for neurodegenerative patients. The text provides a new perspective on the BPMN modeling of Alzheimer’s disease. The presented model offers the option of expanding treatment cost calculation to simulate the process using graphical tools and languages. The overall view of this system creates a much more complex concept of the system and its surroundings.

Covid-19 outbreak led all organizations to reorganize their processes to prevent infection and contagion risk. All healthcare facilities, included penitentiary mental health services, had to redesign their processes to safely deliver care services. In this paper, the case of a Penitentiary Mental Health Division located in southern Italy is presented. Soft System Methodology and Business process management principles and techniques are adopted to analyse and redesign the detainees’ mental health care process. The process, characterized by direct, close and prolonged contact with patients, exposes detainees and healthcare staff to a high Covid-19 infection risk. Through document analysis, interviews with the actors involved in the process and direct observation, the process’s inefficiencies and criticalities are identified. The process is redesigned to make it compliant with Covid-19 prevention provisions and national penitentiary regulations and address the other criticalities. The proposed methodological approach–which innovatively combines Soft System Methodology and Business Process Management–constitutes a human-centered process-based redesign approach that can be used both in healthcare and other organizational settings.
