Process-related predictors of readmissions and mortality following hip fracture surgery: a population-based analysis

Abstract: Aim To study the readmission rates and predictors of readmission following hip fracture surgery in a tertiary care centre with the very short postoperative length of stay. Findings Postoperative length of stay of only 1-2 days did not increase the risk of readmissions. Delay to surgery, prolonged length of stay, not receiving orthogeriatric contribution and discharge to primary rather than secondary care were associated higher readmission rate and/or mortality. Message Although a very short stay in the operati… Show more

“…23 In light of this, routine orthogeriatric screening upon admission of patients with a PFF, as for example widely implemented in the UK, may help to reduce readmission rates in Austria. 24 In line with previous observations, 8,9,17,19 the most common cause for readmission in our study was due to internal medicine-associated complications. Furthermore, implant-associated infections were the most frequent reason for readmission due to implant-related complications in patients with endoprosthetic arthroplasty, mirroring findings in the literature.…”

“…[6][7][8] In order to improve patient care and reduce the logistical, as well as financial burden to healthcare systems, it is important to analyze the factors associated with readmission due to complications developing after surgery for PFFs, together with reasons for readmission. 8,9 However, readmission rates may vary depending on healthcare systems, with differences in health insurance, accessibility to healthcare facilities, and provision and financing of medical treatments, as well as post-discharge care, contributing to the risk. The Austrian healthcare system is one of the most expensive in the European Union, with roughly USAD $5,138 spent on health per capita in 2015.…”

“…O tempo de internação prolongado e o atraso para realizar a cirurgia está muitas vezes associado à reinternação, apesar de que tal relação também pode ser devida à gravidade dos casos. (26) Nesse contexto, a literatura demonstra que pacientes que recebem cuidados multidisciplinares reiternam menos. (27) No comanejo, a situação doméstica, o suporte social, necessidades de reabilitação do paciente e problemas clínicos já são avaliados e encaminhados para uma resolução desde o momento da internação.…”

“…2 We know from multiple studies that hip fractures carry a substantial risk of perioperative and postoperative mortality of approximately 10% at 1 month and 25% at 1 year. [3][4][5][6] Preoperative anemia is common, as is peri operative blood loss, leading to allogeneic blood transfusion rates of 40%-59% in this population. [7][8][9] Blood transfusions are not a benign treatment; infection, length of hospital stay and mortality are all increased by exposure to allogeneic blood.…”
