Prognosis of Dialysed Patients after Kidney Transplant Failure

Abstract: Background/Aims: Patients with a failed kidney transplant represent a unique, high-risk chronic kidney disease population that is increasing in number, and may be sub-optimally managed. Our aim was to compare the survival of patients with failed allografts to patients with native kidney failure and to assess whether their survival is affected by the graft resection. Methods: Kaplan-Meier and Cox-regression survival analyses were performed on the data of 57 patients with graft failure and of 123 transplant-naiv… Show more

“…Previous reports show that the prognosis of patients with graft failure is controversial, with some reporting a poorer prognosis compared with transplant‐naïve patients, while others report a comparable prognosis [4–7]. Considering data from registered patients in Japan [21], the survival rate in this study does not appear to be inferior to that of transplant‐naïve dialysis patients (survival rates at 1, 3, and 5 years were 96.0% vs. 89.9%, 93.8% vs. 76.4%, and 82.4% vs. 60.8%, respectively) [21].…”

“…Some reports suggest that patients undergoing dialysis for graft failure have a poorer prognosis than transplant-naïve patients [4,5]. Conversely, some reports indicate no difference in mortality rates between post-graft failure dialysis patients and transplant-naïve patients [6,7]. Notably, patients undergoing dialysis for graft failure, unplanned initiation, and initiation using a central venous catheter (CVC) are associated with poor outcomes [8,9].…”

“…We believe that repeated kidney transplant recipients should be evaluated separated from the first kidney transplant patients. They are considered as higher risk patients with higher morbidity and mortality rates (17,9) and it is important to evaluate if KDPI can be used as a "risk marker" for graft loss also in the case of repeated transplantation. Our findings suggest that KDPI influences the outcomes of repeated kidney transplantation -higher KDPI is associated with worse graft function and lower graft survival rates.…”

“…Egyes kutatások, köztük egy, a Debreceni Egyetem Klinikai Központjában végzett vizsgálat is, arra az eredményre jutottak, hogy a graftectomizált betegek jobb túléléssel bírnak [8,9].…”

“…Más szerzők statisztikailag jobb vesetúlélést közöl-tek a második transzplantáció előtt nem graftectomizált betegek esetében, de ez a vizsgálat a miénkhez képest jelentősen nagyobb esetszámon alapult [25]. A betegtúl-élést vizsgálva sem kaptunk szignifikáns különbséget a két csoport között, szemben más forrásokkal, ahol azt találták, hogy a graftectomia javítja a retranszplantáció utáni túlélést [8,9]. Saját vizsgálatainknál a trendek egyeztek.…”
