Prognosis of Limitations in Activities in Osteoarthritis of the Hip or Knee: A 3-Year Cohort Study

Abstract: Overall, at the group level, limitations in activities of patients with OA of the hip or knee recruited from hospitals and rehabilitation centers seem fairly stable during the first 3 years of follow-up. However, at the level of individual patients, considerable variation occurs. Prognostic factors for worsening of limitations in activities include increased pain, reduced ROM, and decreased muscle strength at 1-year follow-up; higher morbidity count; and to a lesser extent poor cognitive functioning.

“…However, both studies had lower numbers of participants (Յ48) and there was concern that findings in control knees may be confounded by the contralateral case knees due to the between-knee, withinperson study design and lack of independence of observations in bilateral knees of the same person. For instance, strength in an asymptomatic knee may be reduced by contralateral pain status through a mechanism of central inhibition (40), or through reduced physical activity/function in general (41). However, our current data are in principal agreement with these findings and confirm, in a very large cohort, that pain is a more relevant factor in determining muscle strength than the radiographic disease stage in KOA in both men and women.…”

“…Most of the studies (n = 10) were conducted in a primary care setting. 6,15,16,18,19,27,30,34,39,41 One study recruited patients from a military population, 10 2 studies from rehabilitation centers, 36,42 and 3 studies were community based. 2,21,28 The sample sizes of the studies varied from 54 to 1021 patients, with a mean sample size of 409.…”

“…Radyolojik OA şiddeti arttıkça fiziksel fonksiyonun azaldığı vurgulanmaktadır (11)(12)(13). Dizin fiziksel fonksiyonu beden ağırlı-ğı (14), hareket açıklığı (15), kas aktivasyonu ve propriosepsiyon (4) gibi nöromuskuloskeletal faktörlerle belirlenir. Yaşlı bireylerde, radyolojik OA şiddetine bağımlı olmaksızın fiziksel fonksiyonu belirleyen temel faktörün kas kuvveti olduğu belirtilmekte (11), kas kuvvetindeki azalmanın diz osteoartritini ilerletmekten ziyade insidansı ile ilişkili olduğu vurgulanmaktadır (6).…”
