Prognosis of patients paced for chronic atrioventricular block.

Abstract: Between the years 1960 and 1974, 839 patients were paced for chronic complete atrioventricular block. Analysis of survival compared with the general population showed that 170 deaths were expected according to standard mortality tables and 288 actually occurred, giving a ratio of actual to expected deaths of 1.7:1. Patients with a definite history of myocardial infarction showed a higher than average mortality when paced. Mortality was not influenced whether heart was constant or intermittent, whether the vent… Show more

“…Patients in their eighties paced for complete heart block can expect to survive as long as others of the same age and sex without heart block. 26 Our results suggest that this is so for nonagenarians paced for complete heart block as well. As previously reported,26'27 a history of ischaemic heart disease is important in determining survival after pacing for complete heart block.…”

“…Third‐degree A‐V block is sometimes connected with hypertension, but more frequently with coronary artery disease [16, 17]. However, chronic total A‐V block is associated with fibrous degeneration of the conducting system in 40% of cases [18–24]. This particularly applies in the elderly population [19].…”

“…In chronic third‐degree A‐V block the implantation of a pacemaker is associated with improved prognosis [15, 21]. However, patients under 60 with third‐degree A‐V block have a poorer prognosis than control patients without a block, although this does not apply to older patients.…”

“…A number of clinical reports claim that CHD plays an important role in the development of AV blocks (e.g., Forsberg, 1978;Ginks et a/. , 1979, 1980: Otterstad et ul., 1981Shaw and Kekwick, 1978: Sid- dons, 1974).…”
