Prognostic clinicopathologic factors, including immunologic expression in diffuse large B‐cell lymphomas

Abstract: The aim of this study was to assess the clinical significance and potential prognostic value of the expression of a panel of surface markers, proliferating, suppressor and oncogenic proteins in diffuse large B-cell lymphomas (DLBCL). Biopsies were collected from 158 patients with DLBCL and analyzed immunohistochemically for p53, p21/WAF1, bcl-2, cyclin-D1, bcl-6, mdr, CD5, CD30, epithelial membrane antigen (EMA), Ki-67 and c-myc positive tumor cells. Among these, 76 young and middle-aged patients (20-65 years)… Show more

“…On the other hand, p21 expression was concluded to have no prognostic value for predicting breast cancer-related survival or recurrence-free survival (13). Similarly, survival did not differ significantly among groups with a different p21 labeling index (7) and expression of p21 did not influence overall survival and disease-free survival in diffuse large B-cell lymphoma (22). “…The cell cycle inhibitory activity of p21 has been correlated with its nuclear localization (2). Several studies have evaluated the prognostic significance of p21 in hematopoietic (22,24) and solid (1,7,8,11,13,18,19) tumors, but the prognostic evaluation in MM remains to be explored (6,9,15).…”

“…Multiple studies have looked at the expression of bcl-2 using immunostains and most have found no difference in overall survival. 16,23,[27][28][29][30][31][32][33][34][35] However, some studies have found that bcl-2 expression is associated with a significantly worse overall survival, 3,[36][37][38][39][40][41][42] or event-free survival. [29][30][31]36,37 The discrepancies related to bcl-2 expression and prognosis may be related to the underlying subgroups of DLBCL.…”

“…Expression of Ki-67 has been associated with significantly worse overall survival, 31,46-49 significantly better overall survival, [50][51][52] or to have no effect on overall survival in DLBCL. 16,32,53,54 Although we initially used a cutpoint of 50% for Ki-67, we analyzed all possible cut-points using a survival tree method and found no value that had a statistically significant effect on overall survival or event-free survival. 13 A single study has reported that survivin expression is an independent predictor of worse survival in DLBCL.…”

“…This may have been due to the sensitivity of the PCR-SSCP technique in archival tissue, or to mutations outside exons 5-9. [48][49][50] Expression of p53 protein has previously been demonstrated in 13% to 70% of DLBCLs, 9,12,[50][51][52] and has been frequently demonstrated in the absence of a mutation. 49,53,54 This may alternatively be explained by p53 stabilization by mechanisms such as mdm2 interaction.…”
