AF promotes the progression of Heart failure and preserves Ejection Fraction(HFpEF), so reducing the risk of atrial fibrillation(AF) in HFpEF can significantly improve the prognosis of patients. However, there is lack of research on predictive models of AF in HFpEF. This study aims to build a risk model to predict the risk of AF in HFpEF patients, to guide early intervention of risk factors and reduce the risk of AF. Our study collected and analyzed retrospectively for 259 subjects diagnosed with HFpEF at Renmin Hospital of Wuhan University. Objectives were divided into 2 groups: group Ⅰ: HFpEF with no-AF (n = 128); Group Ⅱ: with AF (n = 131) for the baseline feature analysis. Models were constructed by logistic regression; a nomogram was visualized, and internal validation by bootstrapping, DCA curve was applied the evaluation new model. Compared with non-AF patients, those have older age, faster heart rate, metabolic, disorder, and myocardial. Based on logistic regression forward stepping method analysis, [hyperuricemia (HU)](p < 0.001), [left atrium diameter (LAD)](p = 0.039), [right atrium diameter (RAD)](p < 0.001), [triglyceride(TG)](p = 0.003), [age(> 65years)](p = 0.006), [heart rate(HR)](p = 0.007) were independently predictors of HFpEF with AF. Those were included in this model [area under the curve (AUC) = 0.873] and mean absolute error is 0.01, the threshold probability was within about 0.14 and 0.97 in decision curve analysis (DCA) curve, clinical application by Nomogram provided a greater net benefit. HFpEF patients had AF, older age, RAD, LAD, TG, heart rate and HU are significantly associated with it. The proposed model based on clinical features accurately predicts it and has a good application.

Introduction: Atrial fibrillation (AF) often occurs in patients with rheumatic mitral stenosis (RMS) and is associated with adverse clinical outcomes. Mitral valve mean pressure gradient (MVMPG) is utilized as an indicator to assess the severity of mitral stenosis and its hemodynamic implications. The aim of this study was to investigate the association between MVMPG and AF in individuals with RMS. Methods: We conducted a retrospective analysis of medical records from 360 consecutive patients diagnosed with RMS at the First Affiliated Hospital of Wenzhou Medical University between January 2018 and January 2023. Using both univariate and multivariate logistic regression models, the relationship between MVMPG and AF was evaluated. Restricted cubic splines were employed to test for linearity, and stratified and interaction analyses were performed to evaluate the stability of this relationship among different subgroups. Results: Based on the MVMPG levels, 360 RMS patients in total were categorized into three groups for the analysis: Q1 (<5 mm Hg), Q2 (5–10 mm Hg), and Q3 (>10 mm Hg). The average age was 60.6 years (Q1: 66.1, Q2: 61.9, Q3: 55.8), and 70.8% were female. The prevalence of AF was 39.6%, 56.5%, and 63.2% in Q1, Q2, and Q3, respectively. After adjusting for potential confounders, a significant association between MVMPG and AF was observed. In Q2, there was a 119% increase in AF (OR 2.19, 95% CI: 1.01–4.75), while in Q3, there was a 238% increase (OR 3.38, 95% CI: 1.39–8.19), compared to Q1. The relationship between MVMPG and AF was linear (p = 0.503). These results remained consistent in each subgroup analysis. Conclusion: Our study reveals a significant positive association between MVMPG and AF in patients with RMS, which holds important clinical implications. It is necessary to conduct further research.

Background: hyperglycemia is a trigger for structural alteration of red blood cells (RBCs) and their ability to release extracellular vesicles (EVs). The aim of the study was to elucidate whether glucose control in T2DM patients with concomitant HF and AF affects a circulating number of RBC-derived EVs. Methods: we prospectively included 417 T2DM patients with HF, 51 of them had atrial fibrillation and 25 healthy volunteers and 30 T2DM non-HF individuals. Clinical assessment, echocardiography examination and biomarker measures were performed at the baseline of the study. RBC-derived EVs were determined as CD235a+ PS+ particles by flow cytometry. NT-proBNP levels were measured by ELISA. Results: AF patients with glycosylated hemoglobin (HbA1c) < 6.9% had lower levels of CD235a+ PS+ RBC-derived vesicles than those with HbA1c ≥ 7.0%. There were no significant differences in number of CD235a+ PS+ RBC-derived vesicles between patients in entire cohort and in non-AF sub-cohort with HbA1c < 6.9% and HbA1c ≥ 7.0%, respectively. Multivariate linear regression yielded that CD235a+ PS+ RBC-derived vesicles ≥ 545 particles in µL (OR = 1.06; 95% CI = 1.01–1.11, p = 0.044) independently predicted HbA1c ≥ 7.0%. Conclusions: elevated levels of CD235a+ PS+ RBC-derived EVs independently predicted poor glycaemia control in T2DM patients with HF and AF.
