Abstract. The two types of prognostic factors of papillary thyroid carcinoma (PTC) are static and dynamic. The following static prognostic factors have been conventionally adopted: age, tumor size, extrathyroid extension, lymph node metastasis, and distant metastasis based on pre-, intra-and post-operative findings. These factors are useful to decide therapeutic strategies for PTC patients, including the extent of surgery and radioactive iodine (RAI) ablation. However, even the combination of these factors evaluated pathologically postoperatively is not good enough at predicting recurrence in clinical settings. The dynamic prognostic factors of changes in serum thyroglobulin (Tg) and thyroglobulin antibody (TgAb) values in patients who have undergone a total thyroidectomy are important to evaluate the progression of carcinoma recurrence and to predict patients' cause-specific survival, regardless of their backgrounds and the clinicopathological features of their PTC. Dynamic prognostic factors are superior to static prognostic factors in terms of expressing the condition of recurrence on a real-time basis.Key words: Prognostic factors, Papillary thyroid carcinoma, Thyroglobulin, Thyroglobulin antibody, Lymph node metastasis PAPILLARY THYROID CARCINOMA (PTC) is the most common malignancy originating from the thyroid. Although it is generally indolent, several aggressive features of PTC predict a dire prognosis. The prognostic factors can be classified into two categories, i.e., static and dynamic prognostic factors. Static prognostic factors are based on the patient's background, findings of preoperative imaging studies, intraoperative gross findings, and postoperative pathological findings. They are useful for decisions regarding the choice of therapeutic strategies such as the extent of surgery, radioactive iodine (RAI) ablation and thyroid-stimulating hormone (TSH) suppression. In contrast, dynamic prognostic factors are based on changes over time in serum thyrogloblulin (Tg) and thyroglobulin antibody (TgAb) levels after total thyroidectomy. They are very important to estimate whether and how recurred lesions progress in real time and to predict the cause-specific survival (CSS) of PTC patients. In this review, the static and dynamic prognostic factors for PTC are reviewed separately. Static prognostic factorsStatic prognostic factors can be subdivided into three classifications based on the time of evaluation: pre-, intra-, and post-operation. The former two can be used to decide the extent of thyroidectomy and lymph node dissection.

Rapidly increasing trend of thyroid cancer incidence has turned this disease into a global concern. An estimated number of 64,300 new cases of thyroid cancer occurred in men and women in 2016, which represents 3.8% of all new cancer cases of USA (htps://seer. cancer.gov/statfacts/html/thyro.html). Thus, there is a high possibility for every physician to encounter a case of thyroid cancer during his/her professional lifetime. Thyroid cancer is more prevalent among women and those with positive family history of thyroid disease. Some prior studies have reported a worldwide increasing trend for thyroid cancer incidence in recent decades [4]. Annual percent change (APC) is a commonly used indicator for the assessment of changes in incidence. Based on US National Cancer Institute's Surveillance, Epidemiology, and End Results (SEER) program, it has been reported 2.4% from 1980 to 1997 and 6.6% from 1997 to 2009 in both genders for thyroid cancer (data available at htp://seer.cancer.gov/statfacts/html/thyro.html).© 2017 The Author(s). Table 1 summarizes number of new cases per 100,000 persons by race/ethnicity and sex. A recently published study on adolescent cancer incidence during 1975-2012 has reported a 2.12% increase for females and 1.59% for males between 15 and 19 years of age [5]. However, there is a study indicating that thyroid cancer incidence trend has slowed following release of American Thyroid Association (ATA) guideline in 2009 [6, 7]. It has been reported that overall thyroid cancer incidence had an increasing trend about 8% per year from 2000 to 2009. But APC showed a deceleration about 3% in men and 2.8% in women, which was conined to tumors less than 2.9 cm of size for women [7].Also, thyroid cancer-related mortality was estimated to consist 0.3% of all cancer-related deaths in 2016. A 5-year survival rate of 98.1% has been reported for thyroid cancer. PAPILLARY THYROID CARCINOMA (PTC) is the most common malignancy arising from normal follicular cells. To date, several prognostic factors of PTC evaluated pre-, intra-, or postoperatively have been identified: old age, large tumor size, significant extrathyroid extension, node metastasis, and distant metastasis at the diagnosis [1]. All of these are static factors evaluated at a certain time preoperatively, surgically or pathologically. In contrast, Miyauchi et al. showed that a dynamic prognostic factor -the change in serum thyroglobulin (Tg) levels after total thyroidectomy for thyroglobulin antibody (TgAb)-negative Changes in serum thyroglobulin antibody levels as a dynamic prognostic factor for early-phase recurrence of thyroglobulin antibody-positive papillary thyroid carcinoma after total thyroidectomy Abstract. We demonstrated previously that dynamic prognostic markers such as the thyroglobulin (Tg)-doubling time in thyroglobulin antibody (TgAb)-negative papillary thyroid carcinoma (PTC) and changes in pre-and postoperative TgAb levels in TgAb-positive PTC patients more keenly reflect patients' prognosis than conventional static prognostic factors.Here we investigated periodic changes in TgAb levels in 513 TgAb-positive PTC patients who underwent total thyroidectomy. The TgAb levels at 1 year after surgery decreased to < 50% of the preoperative values in 407 (79%) patients, and the remaining 106 (21%) patients showed no decrease in TgAb. In 426 patients, TgAb was also measured more than 1 year after surgery. Compared with their TgAb levels 1 year after surgery, 59 patients (14%) showed an increase in TgAb levels of > 20% during the follow-up. The postoperative Tg levels at 1 year after surgery remained positive in 44 (9%) patients despite their TgAb positivity. To date (median follow-up period 35 months), 12 of the 426 patients (3%) showed PTC recurrence, and 11 of these patients showed either or both a TgAb elevation later than 1 year after surgery and postoperative Tg positivity. Although further studies with longer follow-ups are necessary, we can conclude that changes in postoperative TgAb levels may be usable as a surrogate tumor marker for TgAb-positive PTC patients after total thyroidectomy.
