Prognostic significance of reactive pulmonary hypertension in patients with chronic heart failure listed for elective heart transplantation

Abstract: A b s t r a c tBackground: Pulmonary hypertension (PH) is recognised in about 60% of patients referred for heart transplantation (HTx), and it influences the patient's prognosis. Aim:To assess the prognostic value of reactive PH in patients listed for elective HTx. Methods:A total of 522 patients were enlisted from the Polish National Registry (POLKARD), listed for HTx, in whom complete haemodynamic data were reported. The endpoint was all-cause death before HTx. Heart transplantation, elective or urgent, was … Show more

“…The combination of: (a) SPAP ≥60 mmHg, (b) TPG ≥12 mmHg, (c) non reversible MPAP (not reduced ≥10 mmHg after administration of vasodilator—iloprost), and (d) PVR ≥3.5 WU was considered as too high risk for Htx and if a patient was in a stable clinical condition for at least 1 month before RHC and had not received new medications or not had the change in existing medications (with exception of diuretics) in the prior 2 weeks and after signing of informed consent a patient was eligible to participate in the study. Diastolic pressure gradient (DPG), defined as diastolic PAP (DPAP)—mean PAWP was used as an additional discriminating index for appropriate selection of patients with HF‐rEF and Cpc‐PH 21 . Baseline biometrical data, characteristics, and medication are described in details in Table 1.…”
