Prognostic Value of N-Terminal Pro-Brain Natriuretic Peptide and High-Sensitivity Troponin T Levels in the Natural History of Transthyretin Amyloid Cardiomyopathy and Their Evolution after Tafamidis Treatment

Abstract: Background: We assesse the evolution and prognostic value of N-terminal pro-brain natriuretic peptide (NT-proBNP) and high-sensitivity troponin T (cTnT-HS) in transthyretin amyloid cardiomyopathy (ATTR-CA) before and after tafamidis treatment. Methods and Results: 454 ATTR-CA patients without tafamidis (Cohort A) and 248 ATTR-CA with tafamidis (Cohort B) were enrolled. Event-free survival (EFS) events were death, heart transplant, or acute heart failure. In Cohort A, 27% of patients maintained NT-proBNP < 3… Show more

“…Moreover, natriuretic peptides are elevated also in other conditions, such as ischaemic heart disease and pulmonary embolism, not being a specific HF biomarker [71,72]. Baseline high sensitivity cardiac troponin T and its increase are associated with mortality, disease worsening and acute decompensation, in HF patients [72][73][74][75][76][77][78][79]. Sensitive contemporary cardiac troponin I is also a predictive biomarker of all-cause and cardiovascular mortality, in HF outpatients [73].…”

“…A clinical benefit of tafamidis treatment, in terms of prolonged major cardiovascular outcome-free survival, lower deterioration of structural and functional changes of the left ventricle, and myocardial work, was also reported outside the frame of clinical trials, in retrospective series of real-life patients [119][120][121][122].…”

“…Cardiac troponin T is known as one of the most sensitive biomarkers for myocardial injury (25). Recently, several studies have demonstrated that high levels of NT-proBNP and cardiac troponin T had a strong predictive ability for CA diagnosis and prognosis prediction (26,27). In our study, both elevated NT-proBNP and cardiac troponin T were found in patients with definite and possible CA, which showed a slightly better diagnostic value compared to native T1 and comparable value with ECV, further supporting the equivalent role of native T1 and ECV as diagnosis indexes.…”
