Progress in rectal cancer staging and treatment

Abstract: Colorectal cancer poses a worldwide major health issue. Rectal cancer has somewhat distanced itself from colonic cancer as a different oncologic entity, due to differences in diagnosis and treatment. Several developments over the last years have improved screening, diagnostics, pre-operative therapy, surgical techniques and postoperative patient care. The multidisciplinary approach to rectal cancer, mainly through the co-operation of surgeons, oncologists and radiologists, seems to be one of the most important… Show more

“…4 Besides the importance of screening, diagnostics, and multidisciplinary treatment of CRC, oncologically adequate and healthy tissue sparing surgery remains the cornerstone of improved survival. 5,6 To determine a total tumor resection intraoperatively, pathologic review of the removed specimen is needed, which increases the duration of the operation and is sometimes not constantly available. 7 Traditional imaging modalities are not suitable to adequately localize oncologically negative resection margins in a real-time intraoperative setting.…”

“…Traditional therapeutic regimens, cytoreductive surgery plus chemotherapy or radiotherapy were applicable for CRC cure and remission [4]. In addition, quality control in surgery and oncological treatment plays a pivotal role in colon cancer therapy, suggesting thqt the combination of modern surgery and new cytostatic agents might contribute to improved five-year survival rates [5]. Heterogeneous molecular mechanisms of CRC were responsible for the difficulty of subsequent treatment [6].…”

“…Some patients respond to neoadjuvant CRT with pathological complete response (pCR) in approximately 20% of casses 1 . Multimodal treatment for rectal cancer includes: introducing effective surgery (total mesorectal excision), neoadjuvant radiotherapy, and modern cytotoxic chemotherapy 2 .…”
