Progression delay in men with mild symptoms of bladder outlet obstruction: a comparative study of phytotherapy and watchful waiting

Abstract: To determine the effect of phytotherapy (Serona repens) on the clinical progression in men with mild symptoms of bladder outlet obstruction (BOO). A total of 189 patients with mild symptoms of BOO, recruited from four different European clinics, were included in the analysis. Age, prostate specific antigen (PSA), international prostate symptom score (IPSS), quality of life (QOL), maximum urinary flow rate (Qmax) and total prostate and transitional zone volume were recorded. Clinical progression was defined as … Show more

“…The burden of BPH on healthcare systems and to society is already high and it is expected to increase in line with the greater life expectancy [4,5]. BPH results from smooth muscle and epithelial proliferation primarily within the prostatic transition zone that can produce LUTS [6-9].…”

“…Data for meta‐analysis were extracted from a total of 27 studies, of which 15 were RCTs and 12 were observational studies. The selected studies included 5 800 patients corresponding to: Permixon ( n = 3 926); α‐blockers ( n = 775; tamsulosin [ n = 377], unspecified [ n = 398]); 5ARIs ( n = 578; finasteride [ n = 484], unspecified [ n = 94]); placebo ( n = 301); control group ( n = 190); and gestonorone caproate ( n = 30).…”

“…Although we know the variables that predict the risk of AUR in patients with BPE, few studies have investigated the variables that can predict which patients, after an episode of AUR, will recover spontaneous micturition or experience a new AUR episode. Assuming a role for prostate inflammation in the “precipitation” of BPE in AUR, it is usual practice to administer not only alpha‐blockers, but also antibiotics and S. repens extract prior to TWOC . This therapeutic strategy is used to eliminate all noxae precipitating AUR (alpha‐adrenergic tonus and prostate inflammation), but poses a risk of standardizing all patients with AUR which, in most cases will have a failure after TWOC.…”
