Progressive Focal Deficit With Porencephaly

Abstract: The association of porencephalic cysts with hydrocephalus was first cited in 1939. Subsequent descriptions of this combination in terms of fixed, nonprogressive neurological problems have been reported. We describe four patients with hydrocephalus who had progressive focal motor deficits secondary to porencephalic diverticulation of the lateral ventricle. Symptoms of increased intracranial pressure were not a consistent finding in all patients. Enlargement of the porencephalic cyst is related to either a gener… Show more

“…The most common clinical signs in human beings associated with these defects are spastic paresis and seizures. Clinical signs may be absent at birth but subsequently occur in an additive fashion (Leahy and Singer 1977) and may not begin for many years (Naef 1958). These pathological conditions have rarely been reported in dogs.…”

“…The pathogenetic hypothesis of intraparenchymal ventricular diverticula is still debatable. 35 Intraparenchymal ventricular diverticula are known to occur in patients with chronic obstructive hydrocephalus without an identifiable focal surgical, vascular or traumatic etiology. 3 Intraparenchymal ventricular diverticulum refers to focal outpouching of the ventricle lined by a prolapsed ependymal layer that typically resolves after treating the hydrocephalus.…”

“…We perform the derivation of the cyst to either the atrium or the peri toneum which produces the disappearance of cranial asymmetry, improvement of the signs of motor deficit and of the ocular motor nerves, a marked improvement in the thickness of the cerebral parenchyma surrounding the CPC by diminishing the intraventricular and intracystic pressure [6,16,18,26,28] (fig. 2, 3).…”

“…We are of the opinion that the mental ability and social behavior are improved since we have read that children not subjected to surgery exhibit a progressive mental deterioration according to the experience of Leahy and Singer [16] …”
