Proliferating trichilemmal tumour: p53 immunoreactivity in association with p27Kip1 over‐expression indicates a low‐grade carcinoma profile

Abstract: The similar p53 immunoreactivity in both PTT and SCCT favour the interpretation of the former as carcinoma, albeit one whose behaviour would be tempered by the well-known regulatory effect exerted by p27Kip1 on the cell cycle.

“…In addition, immunohistochemical staining of Ki‐67 and p53 can further help differentiate proliferating trichilemmal cysts from their malignant counterpart . The expression of p53 limited to the basal layer, as seen in our case, supports the diagnosis of a PTC in contrast to random/diffuse p53 expression observed throughout the malignant PTC or squamous cell carcinoma with trichilemmal differentiation . The presence of CD34 can be used to differentiate trichilemmal cysts from squamous cell carcinoma .…”

“…[8] Proliferation markers like Ki67 and p53 can assist in the stratification of these cases into the three-tier classification as suggested by Ye et al . [79] Ki 67 was found to be only focally positive in <5% of the basal cells in trichilemmal cysts and benign PPT as compared with 30 to 40% cells in malignant PPT by one of the investigators, whereas our case showed Ki67 positivity in 20% of the cells. [7] The present case highlights the importance of these markers in the diagnosis and subtyping of this tumor.…”

“…[56] Alternatively, some authors have recently suggested that PTC is a squamous cell carcinoma. [78910] Nevertheless the malignant potential of these tumors is still debated[11] and new strategies for the evaluation of tumor grade are needed. On the other hand, major association between the presence of ki67 and the grade of malignancy has been observed in all tumors which have been studied by ki67 immunostaining.…”
