Proposed novel nomenclature of vulvar smooth muscle tumors; a case of Smooth Muscle Tumor of Uncertain Malignant Potential (STUMP) of the vulva

Abstract: HighlightsClinical experience with smooth muscle tumors of the vulva is limited. Some tumors present ambiguous histological features concerning for malignancy. These include infiltration, mitotic activity, size, atypia and tumor cell necrosis.A case of smooth muscle tumor of the vulva with cellular atypia is presented.“Smooth Muscle Tumor of Uncertain Malignant Potential” of the vulva is advocated.

“…Smooth muscle tumors that do not fit into these categories are diagnosed as STUMP (2). Most STUMPs are of gynecological origin (especially uterine origin but also extrauterine origin) (3, 4) but scientific literature described rare cases of uroandrological origin. Hsieh et al (5) described, for the first time, a peri-urethral STUMP in a young woman while Zukerman et al (6) reported a rare “atypical” case of bladder STUMP.…”

“…Prognosis and outcome: STUMP is categorized as an intermediate smooth muscle tumor and is reported to be associated with a 7%-27% recurrence rate. 27,29 LEIOMYOSARCOMA OF THE LOWER GENITAL TRACT. Definition: Vulvar leiomyosarcomas account for merely 1% of all primary malignancies of the vulva, yet they represent the most common sarcoma of the vulva.…”

“…In contrast, smooth muscle neoplasms of the vulva are rather uncommon and represent a challenge for diagnosis and treatment. Three categories of vulvar smooth muscle tumors have been described: leiomyomas, atypical leiomyomas, and leiomyosarcomas (1,2).…”
