Propranolol in hypertension during pregnancy.

Abstract: 670BRITISH MEDICAL JOURNAL 2 SEPTEMBER 1978 ventricle and pulmonary artery. Half of all emboli to the right ventricle cause a major complication or death. The use of beta-blocking drugs during pregnancy has been questioned for two main reasons: their possible stimulant action on uterine muscle,1 2 and their pharmacological effects on the fetus.3 In an earlier retrospective study of 10 hypertensive pregnant patients propranolol was used with success.5 We report a prospective study of nine similar high-risk … Show more

“…Their antiarrhythmic effects are primarily due to the (j-adrenoceptor blocking effects, rather than the membranestabilising effects manifested by some of these agents (Singh & Jewitt 1974). 273 Although there is little information regarding their use in the treatment of arrhythmias during pregnancy, a substantial volume of information has accumulated during recent years from studies in pregnant patients with hypertension (Bott-Kanner et al 1980;Eliahou et al 1978;Sandstrom 1978Sandstrom , 1982Tcherdakoff et al 1978), idiopathic hypertrophic subaortic stenosis (Turner et aI. 1968), thyrotoxicosis (Bullock et al 1975Langer et al 1974) and fetal tachycardia.…”

“…Intravenous administration of propranolol has been associated with neonatal hypoglycaemia and respiratory depression (Tunstall, 1969;Cottrill et al, 1977) and in a small retrospective study the use of propranolol was linked with an increased incidence of perinatal death (Lieberman et al, 1978). Prospective studies of the use of orally administered "-blockers do not support these fears (Turner, Oakley and Dixon, 1968;Eliahou et al, 1978;Tcherdakoff et al, 1978). In contrast, hypertensive pregnancies treated with oxprenolol were associated with a reversal of intrauterine growth retardation (Gallery et al, 1978).…”

“…In 1978, Eliahou et al (7) reported the results of treating 25 hypertensive, pregnant patients with propranolol and noted a reduction of predicted fetal wastage. In France, Tcherdakoff et al (41) studied nine patients and noted good blood pressure control and no increase in fetal mortality. However, Liberman and his colleagues (23) also in London, have recently reported their results in treating nine patients with hypertensive pregnancies using propranolol and concluded that the probability of fetal neonatal death was in~ creased by propranolol therapy.…”
