Prospective scenarios: A method of evaluating new decision tools

Abstract: Prospective randomized controlled trials are rarely suitable for the evaluation of new decision making techniques. An approach is described in which a cohort of patients is taken down the usual study pathway to the point at which the new technique would be used. Conventional decision rules are then applied and the results recorded. The new technique is then deployed and the cohort reclassified. The logical and statistical justification for this approach is outlined. More rapid (although possibly less pure) ana… Show more

“…Although computed tomography has been suggested for these patients and may deliver accurate information, its practicability and cost-effectiveness must be challenged. Moreover, for a realistic and pragmatic appraisal of these modalities, it may be more important to assess their ability to influence medical decision-making rather than their ability to diagnose a particular condition [22,23,31]. Such information has not been available until now.…”

“…Analysis of data followed the "prospective scenario" method, as described in detail elsewhere [22,23], where decision tools are evaluated with respect to their ability to diagnose a condition (diagnostic performance) and their ability to influence clinical decision-making (procedural performance). For the purpose of this analysis it was assumed that all patients would have been treated strictly according to the results of the various diagnostic modalities (i.e., the classification following clinical, US, and score evaluations).…”
