Prospective Study of Surgical Treatment of Acromegaly: Effects on Ghrelin, Weight, Adiposity, and Markers of CV Risk

Abstract: Although most markers of CV risk improve with acromegaly remission after surgery, some markers and adiposity increase and are paralleled by a rise in total ghrelin, suggesting that these changes may be related. Understanding the mechanisms and long-term implications of the changes that accompany treatment of acromegaly is important to optimizing management because some aspects of the postoperative profile associate with the increased metabolic and CV risk in other populations.

“…We found, as have others, lower leptin levels in active acromegaly (7,40,41). Our data suggest that observed leptin changes are due primarily to fat mass changes, but are also influenced by leptin’s known sexual dimorphism (7,40,42); in multivariate analysis, gender and fat mass remained significant predictors of leptin levels.…”

“…Our data suggest that observed leptin changes are due primarily to fat mass changes, but are also influenced by leptin’s known sexual dimorphism (7,40,42); in multivariate analysis, gender and fat mass remained significant predictors of leptin levels. In prior studies leptin correlated with fat mass (41), but we found this highly significant only in men.…”

“…In doing this, we attempted to investigate the specific role of acromegaly in the development of atherosclerosis in patients with acromegaly. On the other hand, in a recent study, Reyes-Vidal et al [28] revealed that some cardiovascular risk factors, including body weight and waist circumference, were increased when remission was achieved in acromegaly. In our study, waist circumference in the AA group was higher than the CA group; however, this difference was not statistically significant.…”
