Prostaglandin-Induced Cystoid Macular Edema Following Routine Cataract Extraction

Abstract: To our knowledge, we are reporting the first case of a 59-year-old man who developed recurrent CME with three separate trials of three different prostaglandin class drugs following uncomplicated phacoemulsification with intraocular lens implantation. Despite multiple reports of individual prostaglandin (PG) analogues being suggested as the cause of CME, there are no recommendations regarding withholding these medications in the perioperative period. Our patient first developed CME OD 4-months post uncomplicate… Show more

“…Cystoid macular edema (CME) has been reported in association with the use of latanoprost, travoprost, and bimatoprost 1,2. These drugs are ocular hypotensive lipids, and their side effects can be expected to be somewhat similar.…”

“…33 Cystoid macular oedema is the result of chronic oedema that persists for ≥ 4 months, leading to the formation of cystic honeycomb-like spaces in the retina, 34 and can occur as a consequence of retinal dystrophies, 35 inflammatory diseases (uveitis, scleritis, birdshot chorioretinopathy, toxoplasmosis), 36 retinal vascular disease (retinal vein occlusions, idiopathic retinal telangiectasia, radiation retinopathy), 34 diabetic retinopathy, 34 cataract or other eye surgery, injury to the eye, choroidal tumours, or may be drug induced, for example with prostaglandins such as latanoprost. 37 As with other macular conditions, the main symptoms of cystoid macular oedema are blurred or decreased central vision, but peripheral vision is unaffected. Estimates of the incidence and prevalence of cystoid macular oedema vary considerably depending on the cause of the oedema.…”

“…Of note, endogenous prostaglandins are proinflammatory in nature. They are the key risk factors responsible for CME in patients following cataract surgeries [6]. There are various clinical case reports about the side effect of synthetic prostaglandin analogues (such as latanoprost and travaprost) suggesting a causative link between PGAs and CME in patients, who have undergone both uneventful cataract surgeries and routine cataract extraction [6,7].…”

“…They are the key risk factors responsible for CME in patients following cataract surgeries [6]. There are various clinical case reports about the side effect of synthetic prostaglandin analogues (such as latanoprost and travaprost) suggesting a causative link between PGAs and CME in patients, who have undergone both uneventful cataract surgeries and routine cataract extraction [6,7]. A recent 6-month head-to-head randomized clinical study comparing three different PGAs vs rescula in patients with POAG or OH has strengthened the argument that PGAs pose a potential risk of CME due to the breakdown of bloodaqueous barrier in patients treated with travoprost, bimatoprost, and latanoprost (but not Rescula) [8].…”
